Identification and Management of Perioperative Anaphylaxis
Publication date: Available online 31 May 2019Source: The Journal of Allergy and Clinical Immunology: In PracticeAuthor(s): Gerald W. Volcheck, David L. HepnerThe authors present a case of a patient with multiple episodes of perioperative anaphylaxis. The incidence and the most common causes of perioperative anaphylaxis are reviewed. The most common causes can vary by country and the type of perioperative medications used. The unique environment and the multiple medications and substances used in the anesthesia and surgical setting that make a definitive diagnosis challenging are outlined. A systematic strategy to recogniz...

Acute Management, Diagnosis, and Follow-Up of Suspected Perioperative Hypersensitivity Reactions in Flanders 2001-2018
ConclusionsThis study highlights that there is room for an improved acute management and an optimized diagnostic workup that should not be restricted to patients with severe reactions and/or showing MCA.

Twenty Years' Experience with Anaphylaxis-Like Reactions to Local Anesthetics: Genuine Allergy is Rare
ConclusionsSkin testing and provocative LA challenge are useful to exclude LA allergy, and this testing procedure seems to be appropriate to identify the extremely rare cases with IgE-mediated LA allergy.

Risk of Immediate-Type Allergy to Local Anesthetics Is Overestimated—Results from 5 Years of Provocation Testing in a Danish Allergy Clinic
ConclusionsNone of the 164 patients with suspected immediate-type allergy to LAs reacted on provocation. Thus, no patients have been diagnosed with an immediate allergy to LAs in our regional allergy clinic in the 5-year period studied, and allergy to LAs must be considered very rare. Alternative mechanisms should be considered, but if symptoms are consistent with allergy, other potential allergens should be investigated.

In Vitro Diagnosis of Immediate Drug Hypersensitivity During Anesthesia: A Review of the Literature
Publication date: July–August 2018Source: The Journal of Allergy and Clinical Immunology: In Practice, Volume 6, Issue 4Author(s): Didier G. Ebo, Margaretha Faber, Jessy Elst, Athina L. Van Gasse, Chris H. Bridts, Christel Mertens, Luc S. De Clerck, Margo M. Hagendorens, Vito SabatoQuantification of specific IgE (sIgE) antibodies constitutes an important measure to document anesthesia-related immediate hypersensitivity reactions (IHRs). However, only a few drug-specific assays are available and their predictive value is not known. In cases of non-IgE mediated IHRs, diagnosis might benefit from cellular tests such as baso...
