Usefulness of Triglyceride-glucose index for detecting prevalent atrial fibrillation in a type 2 diabetic population
CONCLUSION: Our data supported a linear and robust correlation between TyG and the prevalent AF in a diabetic population. Moreover, our results implicated the potential usefulness of TyG to refine the detection of prevalent AF in a diabetic population.PMID:36093727 | DOI:10.1080/00325481.2022.2124088

Patient preferences for mitral valve regurgitation treatment: a discrete choice experiment
CONCLUSION: This study of US adults with DMR provides quantitative measures of risk tolerance for tradeoffs related to repair by a beating heart approach relative to conventional open-heart surgery (standard of care). These results may inform DMR treatment choices from regulatory agencies, payers, clinicians, and patients considering a beating heart repair or treatments with similar attributes as potential new alternatives to conventional surgery.PMID:34981982 | DOI:10.1080/00325481.2021.2020571

Perceptions and knowledge gaps on CHA < sub > 2 < /sub > DS < sub > 2 < /sub > -VASc score components: a joint survey of Chinese clinicians and clinical pharmacists
CONCLUSIONS: Chinese medical professionals, especially EPs and GPs, revealed a lack of knowledge and insufficient skills for CHA2DS2-VASc scores and their components. Improvements in the awareness of the CHA2DS2-VASc score and its detailed scoring criteria are urgently needed for Chinese medical professionals. Therefore, education programs concerning the introduction of stroke risk evaluation for AF patients and the development of referencing scoring tools are necessary.PMID:34694951 | DOI:10.1080/00325481.2021.1996815

Sex- related differences in the factors associated with outcomes among patients with strokes of undetermined source: a hospital-based follow-up study.
CONCLUSION: The factors associated with outcomes in SUS differed by sex. For male patients, more severe stroke and hyperlipidemia were associated with poor outcomes in SUS. Risk factors for poor outcomes in female patients were stroke severity, age, and TC level. These findings suggest that taking measures to manage blood lipid levels and severe stroke among patients with SUS are important for both male and female patients and are crucial for reducing the burden of stroke in China. Factors associated with renal impairment in Chinese patients with non-valvular AF and without an established renal disease: a cross-sectional study.
Conclusions: Renal dysfunction is highly prevalent in Chinese NVAF patients and is significantly associated with older age, non-paroxysmal AF, use of ACEI/ARB, congestive heart failure, LVEF <50% and prior TIA/stroke/SE. Further studies on the mechanisms by which these risk factors affect renal function in NVAF patients need to be conducted. PMID: 32174239 [PubMed - as supplied by publisher]

Detecting Nonvalvular Atrial Fibrillation and Anticoagulant Therapy in Cardioembolic Ischemic Stroke.
Authors: Min J, Farooq MU Abstract Nonvalvular Atrial fibrillation (NVAF) is the most common cardiac arrhythmia associated with an increase in risk of stroke and systemic thromboembolism. Strokes related to AF are associated with higher mortality, greater disability, longer hospital stays, and lower chance of being discharged home. The present review will focus on the current status of detecting NVAF and stroke prevention when there is AF. The CHA2DS2-VASc risk stratification scheme is discussed for the identification of patients who are at risk for thromboembolic stroke related to NVAF. Patient with a CHA2DS2-VASc...

Underuse of Anticoagulation in Patients with Atrial Fibrillation.
Authors: Vallakati A, Lewis WR Abstract Atrial fibrillation (AF) is a major risk factor for ischemic stroke. Guidelines recommend anticoagulation for patients with intermediate and high stroke risk (CHA2DS2-VASc score ≥2). Underuse of anticoagulants among eligible patients remains a persistent problem. Evidence demonstrate that the psychology of the fear of causing harm (omission bias) results in physicians' hesitancy to initiate anticoagulation and results in an inaccurate estimation of stroke risk. The American Heart Association (AHA) initiated the Get With The Guidelines-AFIB (GWTG-AFIB) module in June 2013 to...

Implications for your practice: Important changes in the 2014 guideline for the management of patients with atrial fibrillation.
Authors: Littrell R, Flaker G Abstract Atrial fibrillation (AF) is an increasingly common cardiac arrhythmia, currently affecting more than 5 million Americans. Management of patients with AF can be complex, with key strategies including selecting rhythm control versus heart rate control and reducing the patient's risk of stroke or other systemic embolization. The American Heart Association, American College of Cardiology, and Heart Rhythm Society released 2014 Guideline for the Management of Patients with Atrial Fibrillation, which outlines several new recommendations with important clinical implications. Among th...

Management of atrial fibrillation: What is new in the 2014 ACC/AHA/HRS guideline?
Authors: Rajagopalan B, Curtis AB Abstract Abstract Recently, the American College of Cardiology, the American Heart Association, and the Heart Rhythm Society published an updated guideline on the management of atrial fibrillation (AF). This document is a complete revision of the 2006 guideline. Prominent changes in the 2014 guideline include the use of the CHA2DS2-VASc score for risk stratification of stroke, recommendations on when and how to use newer oral anticoagulants for thromboprophylaxis, downgrading of the use of aspirin for thromboprophylaxis of moderate-risk patients, and the use of catheter ablation in...
