Predictors of Stroke in Atrial Fibrillation
We have greatly enjoyed reading the recently published article by Broughton et  al.1 The authors analyzed the data of 4572 participants from the Cardiovascular Health Study and found that the echocardiographic left atrial enlargement detected by echocardiography does not influence the stroke risk observed with atrial fibrillation. However, the safety and efficacy of periprocedural bridging anticoagulation with unfractionated heparin (UH) or low–molecular-weight heparin (LMWH) are still unclear. Identification of Covert Atrial Fibrillation in Cryptogenic Ischemic Stroke: Current Clinical Practice in Japan
A new insertable cardiac monitor, Reveal LINQ (Medtronic, Dublin, Ireland), was approved for clinical use in Japan in March 2016 for detecting atrial fibrillation in patients who develop ischemic stroke with no clearly definable etiology even after extensive workup, so-called cryptogenic ischemic stroke. Cooperation between a specialist of the Japan Stroke Society and a trained cardiologist or cardiac surgeon is needed both for appropriate patient selection and appropriate management of the device.

Systemic Thrombolysis for Ischemic Stroke after Antagonizing Dabigatran with Idarucizumab —A Case Report
We here describe our experience of systemic thrombolysis therapy for severe ischemic stroke in a patient taking dabigatran for atrial fibrillation. After administration of the monoclonal antibody fragment idarucizumab, the anticoagulative effects of dabigatran were efficiently antagonized and application of alteplase was safe and feasible in our patient. This case report may illustrate a novel treatment option in the neurological emergency setting.

Acute Embolic Cerebral Infarction and Coronary Artery Embolism in a Patient with Atrial Fibrillation Caused by Similar Thrombi
We present a case of an 87-year-old female patient who suffered ischemic stroke and myocardial infarction at the same time and received intracranial and coronary thrombectomy. Pathological investigation revealed that thrombi were similar and both infarctions were considered as cardioembolism.

The Influence of Left Atrial Enlargement on the Relationship between Atrial Fibrillation and Stroke
Left atrial enlargement (LAE) is independently associated with an increased risk of stroke and atrial fibrillation (AF). The combination of both LAE and AF possibly increases the risk of stroke beyond that observed with AF.
