Causes of Death in Elderly Patients With Non-Valvular Atrial Fibrillation - Results From the ANAFIE Registry
CONCLUSIONS: This subanalysis described the mortality according to causes of death of Japanese elderly NVAF patients in the DOAC era. Our results imply that a more holistic approach to comorbid conditions and stroke prevention are required in these patients.PMID:36653127 | DOI:10.1253/circj.CJ-22-0614

Epidemiology of arrhythmias and sudden cardiac death in Asia.
Abstract Cardiac arrhythmias are cardiac rhythm disorders that comprise an important epidemiological and public health problem. Cardiac arrhythmias are significantly associated with increased risks of cardiovascular complications and sudden death, consequently leading to decreased quality of life, disability, high mortality, and healthcare expense. Atrial fibrillation (AF) is the most common sustained arrhythmia, and has been further increasing with the aging of society. Although the prevalence is relatively lower in Asians than in Westerners, the prognostic impacts on stroke and mortality in Asians are comparable...

Impact of Renal Dysfunction on Clinical Outcome in Patients With Low Risk of Atrial Fibrillation.
Conclusions: Impaired renal function has an additive prognostic value for thromboembolic events and cardiovascular mortality in low-risk AF patients with CHA2DS2-VASc score 0-1. PMID: 24521763 [PubMed - as supplied by publisher]

Introduction of Point-of-Care Testing in Japanese Outpatient Clinics Is Associated With Improvement in Time in Therapeutic Range in Anticoagulant-Treated Patients.
Conclusions: The introduction of POC testing was associated with an improvement in TTR, mainly through a reduction in the time spent below the target INR. PMID: 24717234 [PubMed - as supplied by publisher]

Hyperuricemia and Transesophageal Echocardiographic Thromboembolic Risk in Patients With Atrial Fibrillation at Clinically Low-Intermediate Risk.
Conclusions: The serum UA level was associated with thromboembolic risk on TEE in patients with nonvalvular AF at low-intermediate risk stratified by clinical risk factors. PMID: 24805815 [PubMed - as supplied by publisher]

Clinical Research and the Development of Medical Therapeutics.
Abstract Clinical research plays a central role in the development of medical therapeutics, but the current system is estimated to take 10-15 years from initial discovery to regulatory approval, at a cost of approximately US$1 billion. Contrast the paths by which 2 anticoagulant options for atrial fibrillation were discovered and ultimately established as treatment options in clinical medicine. Warfarin was discovered by serendipity and compared with placebo in relatively small trials; this was associated with a low cost of development. The new oral anticoagulants were synthesized to provide highly specific, targe...

Latest Advances in Transseptal Structural Heart Interventions.
Abstract Recent advances in structural heart intervention have produced increasing demand for transseptal access, which was first introduced as a diagnostic tool to directly measure left atrial pressure. Transseptal access allows safe and adequate approach to the left atrium and surrounding structures. Percutaneous transcatheter mitral valve repair using the MitraClip device is a safe and less invasive treatment for selected patients with significant mitral regurgitation, who are at high risk for surgery. This is an echocardiographic- and fluoroscopic-guided procedure requiring accurate transseptal access of the l...

Preoperative Tissue Doppler Imaging-Derived Atrial Conduction Time Can Predict Postoperative Atrial Fibrillation in Patients Undergoing Aortic Valve Replacement for Aortic Valve Stenosis.
Conclusions:The PA-TDI duration was an independent predictor of POAF after AVR for AS. Patients with PA-TDI duration >147 ms should be considered high risk and treated appropriately to improve outcomes. PMID: 25030299 [PubMed - as supplied by publisher]

Thromboembolic Events in Paroxysmal vs. Permanent Non-Valvular Atrial Fibrillation.
Conclusions:The crude rate of thromboembolic events was higher in permanent NVAF than in paroxysmal NVAF, but after adjusting for warfarin use and CHA2DS2-VASc score components, paroxysmal and permanent NVAF patients had similar risk of thromboembolism. Impact of Non-Vitamin K Antagonist Oral Anticoagulants on Intracranial Bleeding in Asian Patients With Non-Valvular Atrial Fibrillation.
Abstract The incidence of intracranial bleeding is known to be markedly higher in Japan and other East Asian countries than in countries outside of East Asia. Non-vitamin K antagonist oral anticoagulants (NOACs) have much lower risk of intracranial bleeding than warfarin, so we reviewed the effect of this class of drugs on intracranial bleeding in Asian patients with non-valvular atrial fibrillation (NVAF). Warfarin therapy in Asian or East Asian populations appears to be associated with lower efficacy, poorer safety and a much greater risk of intracranial bleeding when compared with non-Asian or non-East Asian gr...

Bare-Metal vs. Drug-Eluting Stents in Patients With Atrial Fibrillation Undergoing Percutaneous Coronary Intervention.
Conclusions:In real-world patients with AF undergoing PCI, DES use was associated with outcomes comparable to those with BMS without excess bleeding complications. More ST was seen in BMS-treated patients. PMID: 25298168 [PubMed - as supplied by publisher]

Nine-Year Trend of Anticoagulation Use, Thromboembolic Events, and Major Bleeding in Patients With Non-Valvular Atrial Fibrillation - Shinken Database Analysis.
CONCLUSIONS: The 9-year trend of stroke prevention indicated a steady increase of OAC prescription and a partial improvement of TE and MB. Even in the era of DOAC, TE prevention was insufficient in high-risk patients, and DOAC were associated with increased extracranial bleeding. PMID: 26794283 [PubMed - as supplied by publisher]
