Clinical Significance of Fluid-Attenuated Inversion Recovery Vascular Hyperintensities in Transient Ischemic Attack Clinical Sciences

Association Between Left Atrial Abnormality on ECG and Vascular Brain Injury on MRI in the Cardiovascular Health Study Clinical Sciences
Conclusions— ECG left atrial abnormality is associated with vascular brain injury in the absence of documented atrial fibrillation.

Is There an Obesity Paradox for Outcomes in Atrial Fibrillation? Clinical Sciences
Conclusions—There may be an obesity paradox in atrial fibrillation patients, particularly for all-cause and cardiovascular death outcomes. An obesity paradox was also evident for stroke/systemic embolic event outcome in NOAC trials, with a treatment effect favoring NOACs over warfarin for both efficacy and safety that was significant only for normal weight patients.

Factors Associated With Length of Hospitalization in Patients Admitted With Transient Ischemic Attack in United States Clinical Sciences
Conclusions— Approximately 75% of patients admitted with transient ischemic attack stay in the hospital for ≥2 days, with the most important determinants being pre-existing medical comorbidities. Longer duration of hospital stay is associated with 2- to 5-fold greater hospitalization charges.

Hormone Replacement Therapy and Adverse Outcomes in Women With Atrial Fibrillation: An Analysis From the Atrial Fibrillation Follow-Up Investigation of Rhythm Management Trial Brief Reports
Conclusions— HRT does not independently predict mortality, thromboembolism, or bleeding in a large cohort of women with atrial fibrillation.

Body Mass Index and Adverse Outcomes in Elderly Patients With Atrial Fibrillation: The AMADEUS Trial Brief Reports
Conclusion— Obesity was associated with a lower stroke and mortality rate in elderly anticoagulated atrial fibrillation patients. Obesity was related to good quality anticoagulation control.

Current Status and Time Trends of Oral Anticoagulation Use Among Chinese Patients With Nonvalvular Atrial Fibrillation: The Chinese Atrial Fibrillation Registry Study Clinical Sciences
Conclusions— An improvement of OAC use among Chinese patients with AF in Beijing is observed in recent years although only 36.5% of patients with CHA2DS2-VASc score ≥2 received OAC. However, variations between different hospitals were large, suggesting that better education and awareness are needed to improve efforts for stroke prevention among AF patients. Vitamin K Antagonists-Associated Cerebral Hemorrhages: What Are Their Characteristics? Original Contributions
Conclusions— A similar anatomic distribution of ICH in patients with or without VKAs suggests that VKAs should not be considered as a cause of ICH because both locations are usually due to different vasculopathies (deep perforating arteries vasculopathy in deep and cerebral amyloid angiopathy in lobar). The different impact of VKAs on ICH volumes according to location suggests a different susceptibility of these vasculopathies to VKAs. This finding may lead to specific therapeutic strategies.

Hemostatic Therapy in Experimental Intracerebral Hemorrhage Associated With Rivaroxaban Basic Sciences
Conclusions— Prothrombin complex concentrate, Factor VIIa, and fresh frozen plasma prevent excess intracerebral hematoma expansion in a murine ICH model associated with rivaroxaban. The efficacy and safety of this reversal strategy must be further evaluated in clinical studies.

Intracranial Hemorrhage Mortality in Atrial Fibrillation Patients Treated With Dabigatran or Warfarin Clinical Sciences
Conclusions— In this sample of AF patients with ICB on oral anticoagulants, dabigatran was not associated with higher in-hospital mortality compared with warfarin. Hence, reluctance to use dabigatran because of a lack of approved reversal agents is not supported by our results.

Variation in Restarting Antithrombotic Drugs at Hospital Discharge After Intracerebral Hemorrhage Clinical Sciences
Conclusions— The variation in clinical practice and lack of consistent associations with restarting antithrombotic drugs after ICH reflect current knowledge and support the need for randomized controlled trials to resolve this dilemma.
