Assessing the impact of dabigatran and warfarin on health-related quality of life: Results from an RE-LY sub-study
Conclusions: Over the course of one year, all anticoagulated patients without outcome events (e.g. strokes or major bleedings) had stable HRQoL. Scores between dabigatran and warfarin were comparable, which was unexpected given the known complexities of warfarin treatment.

Prevalence and spectrum of PITX2c mutations associated with familial atrial fibrillation
Atrial fibrillation (AF) is the most common form of cardiac arrhythmia seen in clinical practice, accounting for approximately one-third of hospitalizations for heart rhythm disorders. The prevalence of AF is estimated to be 1% in the general population and increases markedly with advancing age, rising from about 0.5% of people in their fifties to nearly 10% of the octogenarians . AF is associated with substantial morbidity and mortality. It confers a 5-fold increased risk of stroke and a double risk of death . AF generally occurs secondary to various cardiac and systemic disorders, including hypertension, coronary artery ...

The role of Toll-like receptor 2 and hypoxia-induced transcription factor-1α in the atrial structural remodeling of non-valvular atrial fibrillation
Atrial fibrillation (AF) is the most common clinically significant cardiac arrhythmia. It is associated with an increased risk of ischemic stroke . A growing evidence suggests a possible association between inflammation and atrial fibrillation (AF) . Atrial biopsies of non-valvular AF patients also have provided the evidence that inflammation may participate in the pathogenesis of AF . Recently, studies have demonstrated the changes in expression of gap junctions and activity of MMPs cause structural changes in the atrium that contribute to the pathogenesis of sustained AF. MMP-9 plays an important role in the degradation ...

Chronic renal disease is associated with stroke and thromboembolism in atrial fibrillation independently from gender
We have read with great interest the article by Guo and colleagues concerning renal dysfunction (defined as the presence of chronic dialysis, renal transplantation or an estimated glomerular filtration rate, eGFR,

Early safety of the Amplatzer Cardiac Plug™ for left atrial appendage occlusion
Conclusions: In this single operator report, LAA occlusion using the double element ACP™ can be safely performed with excellent success rates. Using very specific deployment success, stability and release criteria, this device can achieve LAA occlusion with extremely low complication rates in an extremely frail oral anticoagulant ineligible population with multiple co-morbidities.

Efficacy of different statins for primary prevention of atrial fibrillation in male and female patients: A nationwide population-based cohort study
Atrial fibrillation (AF) is a common arrhythmia and is associated with increased cardiovascular morbidity and mortality . Meta-analysis of randomized controlled trials has showed that statin provides a protective effect against AF . However, different statins may have different levels of efficacy in AF prevention . Statin therapy exhibited different effects between genders in reducing mortality and stroke , and the data on a gender effect of statin in primary prevention of AF is limited. The purpose of this study was to determine the efficacy of different statins for primary prevention of AF in male and female patients.

Atrial Fibrillation reduces the Functional REServe of the Heart by a fifth: A pilot FRESH-AF study
The advent of catheter ablation has reignited the controversy about whether rhythm or rate control should be the preferred mode of therapy for atrial fibrillation (AF) . It is well known that compared to sinus rhythm (SR), AF is associated with increased mortality and morbidity including stroke, heart failure, and impaired quality of life. Furthermore, patients may experience adverse effects from drug treatment that may negate the beneficial effects of maintaining SR . What is still unknown is in what ways and by how much overall cardiac function is impaired when the atrial “kick” in SR is lost after the onset of AF. T...

Anticoagulation with a new oral anticoagulant in heart transplant recipients
Rivaroxaban is now accepted for prevention of stroke and systemic embolism in adult patients with non-valvular atrial fibrillation with one or more risk factors, and treatment and secondary prevention of deep vein thrombosis and pulmonary embolism. Nothing is known about the use of this new anticoagulant in transplanted patients. Here we report a series of heart transplant recipients treated with rivaroxaban and followed at our transplantation center.

Incremental prognostic value of identifying mitral L wave in patients with atrial fibrillation
Atrial fibrillation (AF) is associated with an increased risk of stroke, heart failure and all-cause mortality . AF is frequently associated with left ventricular diastolic dysfunction, resulting in a considerable risk of developing heart failure with preserved ejection fraction . In addition to E-wave and A-wave waves, mitral inflow may have an additional forward flow during mid-diastole, which has been described as a mitral L wave. The appearance of the mitral L wave is reported to be more common in AF than in sinus rhythm and its presence also indicates advanced diastolic dysfunction in patients with and without AF . Ho...

Left atrium in cor triatriatum: Arrhythmogenesis and thrombogenesis leading to stroke
We read with great interest the article by James Ker, where cor triatriatum sinister (CTRS) is considered a potential cause of atrial fibrillation (AF) . This hypothesis is reinforced by the case we describe here.

Fish oil increase the risk of recurrent atrial fibrillation: Result from a meta-analysis
Atrial fibrillation (AF) is a highly prevalent disease in all over the word, which is responsible for reduced quality of life, costly hospitalizations, heart failure, stroke, and death . No current therapy, drug, device, or ablation is uniformly effective, a number of trials and a meta-analysis suggested that we should be satisfied with ventricular rate control and anticoagulation. Although many factors might contribute to these unsatisfactory results, the limited efficacy and the many and dangerous side effects of antiarrhythmic drugs might be particularly responsible for this failure.

Left ventricular geometry and outcomes in patients with atrial fibrillation: The AFFIRM Trial
Conclusion: An easily obtained echocardiographic index of LVH (IVS thickness) may enhance risk stratification of patients with AF, and raise the possibility that LVH regression should be a therapeutic target in this population.

Long-term cardiovascular outcomes in patients with atrial fibrillation and atherothrombosis in the REACH Registry
Abstract: Background: Patients with atrial fibrillation (AF) are at increased risk of thromboembolic events. The long-term prognostic implications of AF in patients with atherothrombosis are unknown. Methods: We compared 4-year CV outcomes in patients with and without a history of AF recorded at their baseline visit in the REACH Registry, an international, prospective cohort of patients with established atherosclerotic arterial disease (CAD, CVD, PAD) or at least 3 risk factors (RFO).Results: AF status and 4year follow-up data were available on 44,518 patients. The prevalence of AF at baseline was 10.3% (n=4582). Overall, p...

Atrial fibrillation: Improvement in identification and stroke preventive therapy —Data from the UK Clinical Practice Research Datalink, 2000–2012
Abstract: Objective: To investigate recent trends in the diagnosis and treatment of atrial fibrillation. Methods: Time trend analysis in the UK Clinical Practice Research Datalink (CPRD —previously the GPRD), 2000–2012.Results: The incidence of AF in men rose from 1.274 (1.271, 1.276) per 1 000 patient years in 2000 to 1.972 (1.969, 1.975) in 2012. In women, it rose from 1.209 (1.207, 1.211) to 1.609 (1.606, 1.611).55847 patients with AF first diagnosed between 2000 and 2012 were included in the study. 54% of men were initiated on anticoagulation therapy in the first year following diagnosis of atrial fibrillation, comp...
