Comparison of Clinical Features, Immune-Inflammatory Markers, and Outcomes Between Patients with Acute In-Hospital and Out-of-Hospital Ischemic Stroke
CONCLUSION: Patients with IHIS had more complicated clinical features, higher levels of immune-inflammatory markers, and higher rates of mortality than patients with OHIS. The most significant predictor for mortality among those with OHIS was NIHSS score >10, and the predictors among patients with IHIS were NLR >5.5 and SII >2120.PMID:35177921 | PMC:PMC8843816 | DOI:10.2147/JIR.S342830

Is the TOAST Classification Suitable for Use in Personalized Medicine in Ischemic Stroke?
J Pers Med. 2022 Mar 19;12(3):496. doi: 10.3390/jpm12030496.ABSTRACTPathophysiologic classification of ischemic stroke is essential to a personalized approach to stroke treatment. The Trial of Org 101072 in Acute Stroke Treatment (TOAST) classification is the most frequently used tool to classify index ischemic strokes. We aimed to assess presence of small and large vessel disease markers across the TOAST groups. In an observational study, 99 ischemic stroke patients were consecutively included and classified according to TOAST. The assessment was supplemented with cerebral small vessel disease (SVD) score, based on Magnet...

Modern issues of improving the prognosis in patients with atrial fibrillation after ischemic stroke
Kardiologiia. 2022 Mar 31;62(3):65-72. doi: 10.18087/cardio.2022.3.n1992.ABSTRACTThe anticoagulant therapy with a priority of direct oral anticoagulants is an approach to the prevention of recurrent stroke in patients with atrial fibrillation (AF) that has presently proved its efficacy and is stated in international clinical guidelines. An extensive evidence-based database demonstrates advantages of rivaroxaban over other drugs of this class in secondary prevention of stroke in AF. Furthermore, these advantages are combined with the optimal safety profile. The rivaroxaban treatment may provide the most favorable prognosis ...

Sex Differences in 90-Day Functional Outcomes for Acute Ischemic Stroke Patients With Obstructive Sleep Apnea
Conclusions Males were found to have two times higher risk for developing poor functional outcomes at 90 days. This may be due to more severe oxygen desaturation, increased susceptibility to oxidative stress, and greater frequency of full airway obstruction in males. Greater emphasis on early diagnosis and treatment of OSA may be necessary to reduce the disproportionate incidence of poor functional outcomes, particularly among apneic male stroke survivors.PMID:37388591 | PMC:PMC10306317 | DOI:10.7759/cureus.39652

A simple score to predict atherosclerotic or embolic intracranial large-vessel occlusion stroke before endovascular treatment
CONCLUSIONS: The atrial fibrillation-blood pressure-clinical neurological deficit-computed tomography hyperdense sign-diabetes mellitus (ABC2D) score can be used to identify atherosclerotic or embolic etiology of patients with ILVOS who require emergency endovascular treatment.PMID:35303701 | DOI:10.3171/2022.1.JNS212924

A case of cardioembloic stroke with wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) syndrome
Rinsho Shinkeigaku. 2022 Jun 24. doi: 10.5692/clinicalneurol.cn-001701. Online ahead of print.ABSTRACTHere, we report a case of an 85-year-old man who presented sudden onset of diplopia, dysarthria, and gait disturbance. On admission, he exhibited bilateral adduction palsy, convergence palsy, and binocular exotropia in the forward gaze showing wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) syndrome. He had a history of chronic nonvalvular atrial fibrillation. DWI-MRI revealed acute ischemic lesions in the paramedian pontine tegmentum, lower midbrain, both cerebellar hemispheres, and left frontal cortex. He was t...

Association of Modifiable Risk Factors with Ischemic Stroke Subtypes in Asian versus Caucasian populations: A Systematic Review and meta-analysis
CONCLUSION: Our findings suggest strong association of smoking, dyslipidemia, and diabetes mellitus with LAA and SVO subtypes in the Caucasian population. However, only diabetes mellitus showed significant association with both LAA and SVO subtypes in Asian population as well. Thus, a majority of the traditional modifiable risk factors had a positive association in LAA and SVO, while a negative protective association was observed in CE subtype, among both the Asian and Caucasian subgroups.PMID:35906805 | DOI:10.1111/eci.13849
