Abstract Ischemic strokes related to atrial fibrillation are highly prevalent, presenting with severe neurologic syndromes and associated with high risk of recurrence. Although advances have been made in both primary and secondary stroke prevention for patients with atrial fibrillation, the long-term risks for stroke recurrence and bleeding complications from antithrombotic treatment remain substantial. Stroke or Transient Ischemic Attack in Patients with Transvenous Pacemaker or Defibrillator and Echocardiographically Detected Patent Foramen Ovale.
CONCLUSIONS: In patients with endocardial leads, the presence of a PFO on routine echocardiography is associated with a substantially increased risk of embolic stroke/TIA. This finding suggests a role of screening for PFOs in patients who require CIEDs, if detected, PFO closure, anticoagulation, or non-vascular lead placement may be considered. Percutaneous Left Atrial Appendage Closure for Stroke Prophylaxis in Patients with Atrial Fibrillation: 2.3 Year Follow-Up of the PROTECT AF Trial.
CONCLUSIONS: The "local" strategy of LAA closure is noninferior to "systemic" anticoagulation with Warfarin. Warfarin Use and the Risk for Stroke and Bleeding in Patients with Atrial Fibrillation Undergoing Dialysis.

The Use of Oral Anticoagulants for Stroke Prevention in Atrial Fibrillation Patients with History of Intra-Cranial Hemorrhage.

Polypharmacy and the Efficacy and Safety of Rivaroxaban versus Warfarin in the Prevention of Stroke in Patients With Nonvalvular Atrial Fibrillation.
CONCLUSIONS: -In a population of patients with AF, two-thirds were on ≥5 medications. Increasing medication use was associated with higher risk of bleeding but not stroke. Periprocedural Stroke and Bleeding Complications in Patients undergoing Catheter Ablation of Atrial Fibrillation with Different Anticoagulation Management: Results from the "COMPARE" Randomized Trial.
CONCLUSIONS: This is the first randomized study showing that performing catheter ablation of AF without warfarin discontinuation reduces the occurrence of peri-procedural stroke and minor bleeding complications when compared to bridging with low molecular weight heparin.
