Routine cerebral embolic protection in transcatheter aortic valve implantation: rationale and design of the randomised British Heart Foundation PROTECT-TAVI trial
EuroIntervention. 2023 Jan 26:EIJ-D-22-00713. doi: 10.4244/EIJ-D-22-00713. Online ahead of print.ABSTRACTTranscatheter aortic valve implantation (TAVI) is an established treatment for aortic stenosis. Cerebral embolic protection (CEP) devices may impact periprocedural stroke by capturing debris destined for the brain. However, there is a lack of high-quality randomised trial evidence supporting the use of CEP during TAVI. The British Heart Foundation (BHF) PROTECT-TAVI trial will address whether the routine use of CEP reduces the incidence of stroke in patients undergoing TAVI. BHF PROTECT-TAVI is a prospective, open-label...

Transcatheter aortic valve implantation with the self-expanding Portico ™ valve system in an all-comers population: procedural and clinical outcomes.
CONCLUSIONS: The PorticoTM TAVI system was safe to implant and achieved a high device success rate. With learning curve effects, the device achieves lower rates of PVL and pacemaker implantation and provides adequate clinical and hemodynamic outcomes through 1 year. Outcomes in nonagenarians undergoing transcatheter aortic valve implantation (TAVI): data from a nationwide analysis.
CONCLUSIONS: Within nonagenarians with AS, patients treated with TAVI had lower risk of cardiovascular events than matched patients treated medically. The patients undergoing a TAVI at this age were often highly selected and the procedure was associated with acceptable long term outcomes. PMID: 31763981 [PubMed - as supplied by publisher]

Repositionable Percutaneous Aortic Valve Implantation with the Lotus Valve: 30-Day and 1-Year Outcomes in 250 High-Risk Surgical Patients.
CONCLUSIONS: Lotus Valve implantation produced excellent valve haemodynamics, minimal PVL, sustained significant improvement in functional status, and good clinical outcomes 1 year post-implant. PMID: 28555592 [PubMed - as supplied by publisher]

One-year outcomes of patients with severe aortic stenosis and an STS-PROM of less than three percent in the SURTAVI trial.
CONCLUSIONS: Among patients at intermediate surgical risk but with STS PROM<3%, TAVI may achieve superior clinical outcomes compared to SAVR. These findings support the need for an adequately powered randomized trial to compare TAVI with SAVR in patients at low operative risk. PMID: 29992904 [PubMed - as supplied by publisher]

Transcatheter Aortic Valve Implantation using ACURATE neo in bicuspid and tricuspid aortic valve stenosis: a propensity matched analysis of an European experience.
CONCLUSIONS: this preliminary analysis shows that the use of ACURATE neo in bicuspid aortic valves is feasible and has acceptable 30-days outcome. Larger studies are needed to confirm our preliminary findings. PMID: 30375336 [PubMed - as supplied by publisher]

Early Hemodynamic Changes and Long-Term Outcome Of Patients With Severe Low Gradient Aortic Stenosis After Transcatheter Aortic Valve Replacement.
CONCLUSIONS: NFLG-AS was the most prevalent form of LG severe AS and was associated with adequate left ventricular compensation and good prognosis. On the other hand, CLFLG-AS represents the heart-failure with reduced ejection fraction (HFrEF) form of AS and was associated with the worst prognosis, whereas PLFLG-AS represents the heart-failure with preserved ejection fraction (HFpEF) form of AS with intermediary prognosis. Both groups showed early hemodynamic reverse response after TAVR. PMID: 31566569 [PubMed - as supplied by publisher]

Transaxillary compared with transcarotid access for TAVR: a propensity-matched comparison from a French multicenter registry.
CONCLUSIONS: One-month mortality, 1-month stroke/TIA and 1-year mortality are similar with TAx and TC TAVR. However TC-TAVR is accompanied by more minor bleeding and main access hematoma compared with the transaxillary route. PMID: 32310129 [PubMed - as supplied by publisher]

Clinical Outcomes of TAVI or SAVR in Men and Women with Aortic Stenosis at Intermediate Operative Risk: a post hoc analysis of the randomized SURTAVI trial.
CONCLUSIONS: Aortic valve replacement, either by surgical or transcatheter approach, appears similarly effective and safe for males and females at intermediate surgical risk. Functional status appears to improve most in females after TAVI. PMID: 32715995 [PubMed - as supplied by publisher]

Thirty-day and one-year outcomes of the Navitor transcatheter heart valve in patients with aortic stenosis: the prospective, multicentre, global PORTICO NG Study
CONCLUSIONS: The PORTICO NG Study demonstrates low rates of adverse events and PVL up to 1 year in patients at high or extreme surgical risk, confirming the safety and efficacy of the Navitor THV system.PMID:36895190 | DOI:10.4244/EIJ-D-22-01108

Anaesthetic management of transcatheter aortic valve implantation: results from the Italian CoreValve registry.
CONCLUSIONS: Our study indicates that, in experienced centres which have gone beyond their initial learning curve with TAVI, the use of local anaesthesia in a selected patient population can be associated with good clinical outcomes. Nevertheless, as severe procedural complications are possible, an anaesthesiologist should always be present as part of the team. PMID: 25772903 [PubMed - as supplied by publisher]

Preliminary outcomes after transcatheter aortic valve implantation in patients with systemic sclerosis.
CONCLUSIONS: The present case series suggests that the TAVI procedure is feasible in patients with SSc and severe AS. PMID: 25912392 [PubMed - in process]
