Even Faster Door-to-Alteplase Times and Associated Outcomes in Acute Ischemic Stroke
We report a single-center, retrospective assessment of the safety and efficacy of alteplase treatment within 45 minutes. Methods: Five hundred and eighty-six patients were treated with alteplase in our emergency departments (EDs) between January 2014 and October 2016; 368 patients were included for analysis.

Anatomical distribution of lower-extremity deep venous thrombosis in patients with acute stroke
Post-stroke paralysis is a common contributor to deep vein thrombosis (DVT) of the lower extremities. DVT of the lower extremities can spread to the proximal circulation, and the detachment of the thrombus may result in fatal pulmonary embolism. Therefore, the early diagnosis and timely treatment of DVT should be highlighted.1 DVT can occur in various sites in deep veins of the lower extremities, and the clinical risks and therapeutic guidelines for thrombus in different anatomical locations considerably varies.

Response to “Measuring the Optic Nerve Sheath Diameter with Ultrasound in Acute Middle Cerebral Artery Stroke Patients”: For Application to Long-Duration Spaceflight (LDSF)
We thank the authors of the study for their contribution to literature and their insights into the utilisation of optic nerve sheath diameter (ONSD) measurements with regards to therapeutic efficacy of thrombolytic therapy (TT) and identification of complications in populations post-MCA stroke. We would like to introduce the discussion of potential translational benefits of this methodology to the field of long-duration spaceflight (LDSF).

Acute Ischemic Stroke Revealing ChAdOx1 nCov-19 Vaccine-Induced Immune Thrombotic Thrombocytopenia: Impact on Recanalization Strategy
We describe a case of a young healthy women suffering from acute ischemic stroke due to large vessel occlusion without cerebral venous thrombosis 8 days after vaccination and its consequences on recanalization strategy.Considering the thrombocytopenia, intravenous thrombolysis was contraindicated. She underwent mechanical thrombectomy with complete recanalization and dramatically improved clinically.

Prediction of Intracerebral Hemorrhage After Endovascular Treatment of Acute Ischemic Stroke: Combining Quantitative Parameters on Dual-Energy CT with Clinical Related Factors
At present, intravenous thrombolysis and endovascular therapy are the two main treatment methods of acute ischemic stroke (AIS). The application of intravenous thrombolytic therapy is limited by strict time window restrictions, while endovascular therapy significantly increases the rate of revascularization of arterial occlusion and can improve the clinical prognosis of patients, so it is increasingly applied in clinical practice.1 Despite advances in patient selection for reperfusion therapy for AIS, intracerebral hemorrhage (ICH) remains a serious potential complication, and often occurred within 72  h after surgery.

Thrombectomy, along with intravenous alteplase, had been the standard medical care to achieve rapid and complete reperfusion for acute ischemic stroke.1 Many devices and techniques had been developed and devised, respectively, to achieve rapid and complete reperfusion. Thrombectomy includes stent retriever thrombectomy, with or without aspiration assistance, and contact aspiration (CA), mainly a direct aspiration first-pass technique (ADAPT). Kang et al. reported CA using a catheter with an inner diameter (I.D.) of 0.041 in.

Simultaneously Occurring Ischemic Stroke, Leg Artery Occlusion, and Pulmonary Embolism Induced by a Uterine Myoma
Paradoxical embolism from venous thrombosis through the patent foramen ovale is a rare but well-known cause of stroke in young adults. Here, we report a case of simultaneous middle cerebral artery infarction, multiple occlusions of the leg arteries, and pulmonary thromboembolism from the venous thrombus, all due to compression of the external iliac vein by a uterine leiomyoma.
