Effect of drug use in the treatment of acute ischemic stroke: A scoping review
CONCLUSION: Evidence for thrombolytic and thrombectomy treatment in drug users remains limited. Controlled studies are needed to examine complication profile and outcomes following thrombolytic and thrombectomy treatment in this population.PMID:36128166 | PMC:PMC9479649 | DOI:10.25259/SNI_561_2022

O10 The necessity of monitoring platelet response and adjusting antiplatelet agents for endovascular treatment of unruptured intracranial aneurysms: a single-center, retrospective, cohort study
ConclusionActive monitoring and adjustment antiplatelet agents did not reduce ischemic complications in endovascular treatment; however, tended to reduce the ischemic complication, especially in flow diversion. Low-dose prasugrel may be feasible without increasing hemorrhage in the endovascular treatment of unruptured intracranial aneurysms.ReferencesLi W. O23 Mechanical thrombectomy using a modified ADAPT (a direct aspiration first pass technique) in a patient with m1-occlusion and concomitant mca-aneurysm
We present a case in which a patient with a right-sided MCA M1 occlusion (NIHSS 30) and concomitant mediabifurcation aneurysm was fully recanalized (TICI 3) using a modified ADAPT technique, without prior passage of the thrombus as previously described by Turc et al.2. To us, the avoidance of blind aneurysm passage by microwire and microcatheter appears to be a major advantage of the method used. Instead, the aspiration catheter was advanced via a 0.035 Terumo guide wire for proximal stabilization. Thrombus was aspirated with proximally inflated BGC and repeated imaging series up to visualization of the aneurysm neck. Thus...

P61 Double stent-retriever as first-line approach in mechanical thrombectomy - a randomized in vitro evaluation
ConclusionsUnder certain anatomical conditions, the double-SR technique combined with distal aspiration may induce a higher%FPE than single-SR. The perks of using double-SR as the primary approach are highlighted when treating saddle occlusions.ReferencesKlisch J, Sychra V, Strasilla C, et al. P74 Keep it simple: MAC (Manual Aspiration with Contrast) technique for the endovascular management of acute ischemic stroke (LVO & MVO)
We report a prospectively collected clinical experience with a simple technique: MAC (Manual Aspiration Contrast Enhancement) as the primary method for vessel recanalization.Gently contrast injection while the aspiration catheter is advancedto the thrombus and subsequentlycreating a closed-loop system with the contrast column within the cathetercan result in better visualization during the aspiration, thus improving the FPEavoiding clot fragmentation, multiple passes and blind movements of catheter by a single operator.Methods47 prospectively patients with ELVO and 3 cases of middle vessel occlusion (MVO) at four instituti...

Cerebral venous thrombosis after COVID-19 vaccination: the role for endovascular treatment
Cerebral venous thrombosis (CVT) is a unique cause of stroke. Population-based studies demonstrate an incidence of clinically significant CVT of 1.3–1.5 per 100 000 per year.1 2 This may be an underestimated incidence, as these studies were not based on review of imaging and the often benign nature of this entity may lead to underdiagnoses. In the JNIS paper published by Cleaver et al,3 the authors describe their experience with endovascular treatment (EVT) in patients with cerebral venous sinus thrombosis in patients following COVID-19 vaccination. CVT-related stroke primarily affects young adult and middle-aged pat...

O-012 Peri-procedural complications, clinical outcomes and recurrence/retreatment rates after endovascular versus surgical treatment of intracranial aneurysms: a real-world assessment from the NVQI-QOD cerebral aneurysm registry
ConclusionEndovascular treatment of intracranial aneurysms was associated with better overall clinical outcomes than surgical clipping with improved disability free survival and better disposition at discharge or follow-up, across both ruptured and unruptured aneurysms. Although intraoperative complications were higher for ruptured aneurysms treated with endovascular treatment, there was no difference in unruptured aneurysms versus surgical clipping. Post-operative complications were lower for the endovascular cohort regardless of presentation. Overall, these data support ongoing primary adoption of endovascular treatment ...
