Risk of Bleeding from Antithrombotic Agents (AT) in Patients with Cerebral Cavernous Malformations (CCMs). (I2-1.002)
CONCLUSIONS: There is no apparent increase in the rate of hemorrhage in CCM patients while taking AT, although the small number of patients may limit the power to detect a difference. Helping Haiti: Addressing the Burden of Neurologic Disease (I10-2.004)

Clinical Reasoning: Acute-onset homonymous hemianopia with hyperglycemia: Seeing is believing
A 32-year-old woman with a history of bipolar disorder, polycystic ovarian syndrome, and hypertension and a 4-year history of uncontrolled type 2 diabetes mellitus (DM) presented with bifrontal headache, elevated blood sugars (>500 mg/dL), and abrupt-onset left homonymous hemianopia upon awakening. Vital signs included temperature 98.0°F, blood pressure 160/89 mm Hg, and heart rate 67 bpm. Neurologic examination showed dense left homonymous hemianopia with macular sparing and without other focal findings.

A 26-year-old woman developed headache, diplopia, and pulsatile tinnitus in 10 days. Neurologic examination revealed limitation of left eye movement, left dilated pupil with optic disc swelling and mild decreased vision, and a bruit over the left superior orbit. Clinical Reasoning: A 24-year-old woman with progressive headache and somnolence
A 24-year-old woman presented with progressive somnolence and headache following 2 days of nausea and vomiting. She had a history of developmental delay, attention-deficit disorder, and remote seizures. Medications included combined estrogen-progestin oral contraceptives. On presentation, she was afebrile, somnolent but arousable, groaning incoherently, and unable to follow commands. Optic disc margins were blurred bilaterally. Gaze was midline and deviated downward with restricted spontaneous upward gaze but full lateral gaze. She moved the right side less briskly than the left.

Multimorbidity and the risk of restless legs syndrome in 2 prospective cohort studies
Conclusions: Multimorbidity was a strong risk factor for RLS in these 2 population-based cohort studies. The results support the hypothesis that cumulative disease burden is more important than the presence of a specific single disease in the pathophysiology of RLS.

Rapid development of 9 cerebral aneurysms in varicella-zoster virus vasculopathy
A patient with zoster in multiple dermatomes, severe headache, and normal magnetic resonance angiography (MRA) and 4-vessel digital subtraction angiogram (DSA) developed 9 anterior circulation aneurysms 2 months later. Antiviral treatment resulted in clinical improvement, size reduction of most aneurysms, and complete resolution of the 2 largest aneurysms.

Reversible cerebral vasoconstriction syndrome following carotid stenting
A 60-year-old woman on venlafaxine presented with headache and left-sided weakness 1 month after undergoing right common carotid artery stenting. Catheter-based angiogram identified new irregularities of the right anterior cerebral and right middle cerebral artery (figure, A–C). Clinical Reasoning: A 42-year-old man who developed blurred vision and dropped his iPod while jogging
A 42-year-old man noted sudden onset of blurriness in his left eye and dropped his iPod from his right hand while jogging. In the emergency room, it was noted that visual blurring resolved with right eye closure, but his ophthalmologic examination was otherwise normal. He had subtle right nasolabial fold flattening and right arm pronator drift. His examination was otherwise normal. He reported no headache, neck pain, prior trauma, prior transient neurologic deficit, or palpitations. He took no medications and did not smoke, drink alcohol, or use illicit drugs.

Cerebrovascular regulation, exercise, and mild traumatic brain injury
A substantial number of people who sustain a mild traumatic brain injury report persistent symptoms. Most common among these symptoms are headache, dizziness, and cognitive difficulties. One possible contributor to sustained symptoms may be compromised cerebrovascular regulation. Reduced trigeminovascular cyclicity in patients with menstrually related migraine
