Hemiarch and Total Arch Surgery in Patients With Previous Repair of Acute Type I Aortic Dissection.
CONCLUSIONS: In patients with previous acute type I aortic dissection repair, hemiarch and total arch operations have respectable morbidity and survival rates. Congestive heart failure predicts operative death, long-term mortality, and our adverse event endpoint. What is the best approach in a patient with a failed aortic bioprosthetic valve: transcatheter aortic valve replacement or redo aortic valve replacement?
A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was whether transcatheter aortic valve-in-valve replacement (viv-TAVR) or redo aortic valve replacement (rAVR) is the best strategy in a patient with a degenerative bioprosthetic aortic valve. Altogether, 162 papers were found using the reported search, of which 12 represented the best evidence to answer the question. Does preoperative statin therapy prevent postoperative atrial fibrillation in patients undergoing cardiac surgery?
In conclusion, the available evidence suggests that preoperative statin therapy in patients undergoing elective cardiac surgery is associated with the following: (1) a lower incidence and risk of developing postoperative AF, (2) reduced stroke, (3) a shorter hospital stay and (4) reduced levels of inflammatory markers postoperatively. However, while the evidence supports the use of statins preoperatively, the optimal duration, dose and type of statin cannot be concluded from this review.

Postoperative Atrial Fibrillation After Thoracic Aortic Surgery
Conclusions Several risk factors contribute to the incidence of POAF after thoracic aortic surgery. We found that POAF significantly increased 30-day operative mortality (p < 0.0001). Our findings can be used to develop a risk stratification system for the prediction of POAF.

Surgical Ventricular Restoration: Is There Any Difference in Outcome Between Anterior and Posterior Remodeling?

A More Specific Anticoagulation Regimen Is Required for Patients After the Cox-Maze Procedure
Conclusions Our results indicate that the decision to discontinue OAC after the Cox-Maze procedure should not be based solely on CHADS2 scores; rather, rhythm status, echocardiographic findings, and patient risk for bleeding should be considered. These findings underscore the need for an OAC protocol for patients who have undergone the Cox-Maze procedure with appropriate LAA management.

089 * innominate artery cannulation for proximal aortic surgery: outcomes and neurologic events in 263 patients
Conclusion: Innominate artery cannulation can be performed safely and poses a low risk of neurologic events in procedures requiring hypothermic circulatory arrest. This artery may be considered the optimal perfusion site for delivering ACP.
