The impact of prior cardiac surgery on patients undergoing surgical repair for acute type A aortic dissection
ConclusionsThese findings suggest that re-sternotomy can be safely performed with similar outcomes as first-time sternotomy. Central initiation of CPB after sternal reentry limits CPB time and may therefore represent a protective strategy that enhances outcomes for patients presenting with ATAAD and prior cardiac surgery.

Trans ‐ventricular catheter device‐based closure of postmyocardial infarction ventricular septal defect following coronary artery bypass grafting: A staged hybrid approach
AbstractA 66 ‐year‐old woman with a history of hypertension, ischemic stroke, and rheumatoid arthritis presented to the hospital with severe angina pectoris and dyspnea and was diagnosed with myocardial infarction (MI). Coronary angiography revealed multisystem coronary artery occlusive disease. Due to refra ctory myocardial ischemia/evolving MI, emergency coronary artery bypass grafting (CABG) was undertaken. Intraoperative transesophageal echocardiography additionally revealed an apical muscular ventricular septal defect (VSD). Concomitant VSD repair was deferred due to the absence of surface evidence of transmural M...

Risks and benefits of concurrent treatment with antiplatelet and anticoagulation therapy in post ‐op bioprosthetic aortic valve replacement patients
ConclusionPatients who underwent bAVR and were discharged on concurrent therapy compared to aspirin only had a significantly increased risk of bleeding without a significant difference in thromboembolic events.

Developing a cardiac surgery unit in the Caribbean: A reflection
ConclusionFrequent outside visits complemented by training in an overseas center, and transfer of knowledge proved to be an effective strategy to develop a cardiac surgery unit in an emerging country with results comparable to accepted international standards.

Effects of antidiabetic medications on cardiovascular outcomes
ConclusionOptimizing blood glucose level during the perioperative period with correct medication and dose have a significant role in reducing morbidities. Measures should be taken to provide a safe blood glucose level for optimum outcomes.

Type A aortic dissection in the East and West: A comparative study between two hospitals from China and the US
ConclusionsThere was a significant increase in the number of ATAAD at NDTH during the study period while the number at MGH remained stable. Hypertension was a common major risk factor; however, the onset of ATAAD at NDTH was nearly one decade earlier than MGH. Chinese patients tended to have more complicated preoperative pathophysiology at presentation and underwent more extensive surgical repair.

Sodium nitroprusside versus nicardipine for hypertension management after surgery: A systematic review and meta ‐analysis
ConclusionNicardipine and sodium nitroprusside have similar abilities in reducing afterload in the postoperative cardiac population. Statistically significant differences were found in pulmonary artery pressure and cardiac index. It may be beneficial to consider nicardipine for afterload reduction in patients with a low cardiac index.

Effects and outcomes of cardiac surgery in patients with cardiometabolic syndrome
ConclusionsPatients with CMS were more likely to present with increased comorbidities. Patients with CMS undergoing CABG were at risk for worse short ‐term secondary postoperative outcomes and reduced long‐term survival. The data supports the need for further investigation for risk reduction surrounding operative revascularization.

Aortic valve replacement with pulmonary hypertension: Meta ‐analysis of 70 676 patients
ConclusionsPatients with severe AS and severe PHT had a significant increase in operative mortality and more than double the risk of long ‐term mortality following SAVR compared with patients with no PHT. Such patients may benefit from a less invasive transcatheter aortic valve intervention.

Predicting postoperative atrial fibrillation after myocardial revascularization without cardiopulmonary bypass: A retrospective cohort study
ConclusionOff ‐pump coronary artery bypass grafting proved a safe and effective procedure, with low postoperative atrial fibrillation occurrence, for myocardial revascularization. Chronic renal insufficiency and the use of venous‐origin graft solely proved to be independent predictor factors for PAOF.
