Elevated Blood Pressure Before Left Ventricular Assist Device Support Implant is Associated with an Increased Risk of Stroke
Despite the recent advances in Left Ventricular Assist Device (LVAD) therapy, stroke remains a frequent and catastrophic complication. Hypertension (HTN) after LVAD implant has been associated with an increased risk of stroke, but the association between HTN before LVAD implant and stroke has never been studied. We hypothesized that LVAD patients with elevated blood pressure (BP) during outpatient visits before the LVAD implant may have an increased risk of stroke.

RVOT-VTI/PASP Is a Novel Noninvasive Parameter of Pulmonary Artery Compliance and Improves After Treatment with Pulmonary Hypertension-Specific Therapy
Pulmonary artery compliance (PAC) is an independent predictor of mortality in patients with pulmonary hypertension (PH). We have previously shown that the ratio of echocardiography derived right ventricular outflow tract velocity time integral (RVOT-VTI; an estimate of stroke volume) to pulmonary artery systolic pressure (PASP) correlates with PAC measured by right heart catheterization. We sought to determine whether PH-specific therapy improves PAC in patients with pulmonary arterial hypertension (PAH).

Hypertension, Arterial Compliance and LVAD Pump Function
Hypertension (HTN) has been implicated as a risk factor for stroke in continuous-flow Left Ventricular Assist Device (LVAD) populations. Active treatment of HTN has been shown to decrease cerebrovascular accident rates in the recent trial. Current guidelines recommend a mean arterial pressure (MAP) of 80mmHg in LVAD patients; however, it is difficult to reliably measure Blood Pressure (BP) in this cohort. Increased BP will further affect pump output since LVAD flows are altered by both preload and afterload.

Prolonged Circulatory Arrest is Not Associated with Stroke or Mortality after Pulmonary Thromboendarterectomy
Patients with chronic thromboembolic pulmonary hypertension (CTEPH) frequently present with extensive distal clot burden, which adds complexity to pulmonary thromboendarterectomy (PTE). Complete endarterectomy often necessitates prolonged periods of hypothermic circulatory arrest (HCA), the ramifications of which are poorly understood. Objectives were to describe the association of HCA time with post-operative neurologic complications, operative mortality and other major morbidity after PTE.
