Red and White Thrombus Characteristics in Patients Undergoing Carotid Endarterectomy
Ischemic heart disease and stroke, which are associated with atherosclerosis in the coronary and carotid arteries, accounted for one-third of deaths worldwide.1 The stroke type can be ischemic or hemorrhagic. Hemorrhagic stroke is divided into intracerebral and subarachnoid hemorrhage types. Ischemic stroke occurs because of small vessel occlusion, large artery atherosclerosis, cardiogenic embolism, and other causes.2 Ischemic stroke and transient ischemic attack (TIA) usually arise from unstable carotid lesions, which cause thrombus formation or carotid artery occlusion.

Development of Internal Carotid Artery Dissection During Masturbation
Sexual intercourse is known as one of the daily activities triggering spontaneous cervicocephalic artery dissection (sCAD), however, it has been unclear if masturbation can trigger the development of sCAD. Herein, we report a case of sCAD in association with masturbation. A 51-year-old right-handed man developed subarachnoid hemorrhage during masturbation. The dissection of the left internal carotid artery was evident on the 9th hospital day. Finally, he was treated with stenting and coiling and discharged with a good prognosis.

Acute Cerebrovascular Disorders and Vasculopathies associated with Significant Mortality in SARS-CoV-2 patients admitted to the Intensive Care Unit in The New York Epicenter
We present a case series of consecutive COVID-19 patients with cerebrovascular disease treated at our institution including 3 cases of cerebral artery dissection including subarachnoid hemorrhage. Knowledge of the varied presentations including dissections will help treating clinicians at the bedside monitor and manage these complications preemptively.

Aneurysmal subarachnoid hemorrhage (aSAH) is both a hypercoagulable and inflammation state in which many biomarkers have been studied. Activated platelets have been identified to be of clinical importance in thrombosis and neuroinflammation after aSAH. The aim of this study was to investigate the relationship between mean platelet volume (MPV) to platelet count (PC) ratio, a surrogate parameter for activated platelets, and the functional outcome in aSAH patients.

Rules About Rules – The 6-h CT Rule For Subarachnoid Hemorrhage
In this issue of the Journal of Stroke and Cerebrovascular Diseases, Chakraborty and colleagues describe the cautionary tale of a patient who presented with a thunderclap headache concerning for a subarachnoid hemorrhage (SAH). A head computed tomographic scan (CT) done 4  h after the onset of the headache was interpreted as negative but the patient was ultimately found to have an aneurysmal SAH.1

Characteristics of a Diverse Cohort of Stroke Patients with SARS-CoV-2 and Outcome by Sex
Coronavirus disease 2019 (COVID-19), caused by the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), usually presents as a respiratory illness. Neurological manifestations can be seen in 36.4% of patients.1 Patients with vascular risk factors (VRFs), including history of stroke, tend to have worse prognosis.2 COVID-19 triggers a robust inflammatory response which leads to hypercoagulability and thromboembolism.3 Reports of stroke in patients with COVID-19 are mostly limited to small case series or case reports of ischemic stroke (IS), though intracerebral hemorrhage (ICH) and subarachnoid hemorrhage (SAH) have also been reported.

Relative Alpha Variability Changes Precede Alpha-Delta Ratio Changes in Cerebral Ischemia
We present a case of aneurysmal subarachnoid hemorrhage complicated by vasospasm as detected by the cerebral angiogram. The patient was being monitored on electroencephalogram. It showed early signs of cerebral ischemia represented by decline in the Alpha-Delta-Ratio (ADR) and the Relative-Alpha-Variability (RAV). Surprisingly, the RAV changes preceded the ADR changes. This is a significant finding that can also apply to early reocclusion or reperfusion injuries after mechanical thrombectomy.

Subarachnoid hemorrhage (SAH) results in neurocognitive dysfunction and anxiety in humans and in animal models. Spontaneous Subarachnoid Haemorrhage as a Cause of Out-of-Hospital Death
Spontaneous subarachnoid haemorrhage (SAH) is a neurological emergency with high mortality and morbidity. The overall incidence rate has declined over the last centuries, but is still 6.3 per 100.000 persons per year in Europe.1 Older studies found that 12% of patients with SAH died before they reached medical attention.2,3 Prehospital systems have undergone major changes in recent years, but we have been unable to identify any recent European studies that could elucidate if these numbers remain stable.
