Teaching NeuroImages: Fetal deep medullary vein thrombosis presenting as progressive intracerebral hemorrhage
A 27-year-old pregnant woman underwent ultrasonography at 36 weeks gestation, which revealed unilateral echogenic lesion (figure 1). Her previous week's scan was normal. Possibilities of intracerebral hemorrhage and neoplasm were considered. The neonate, delivered 7 days later, had seizures on day 1. The MRI revealed bilateral extensive supratentorial intracerebral hemorrhages (figure 2). The baby died 3 days later due to continuing seizures and raised intracranial pressure. The MRI was characteristic of deep medullary vein (DMV) thrombosis with secondary hemorrhagic infarction. Prothrombotic workup revealed antithrombin I...

Pre-hospital emergency skills for medical students
Conclusion These results show that there is some anxiety amongst students about being able to manage pre-hospital emergencies in which the public might expect them to cope or in which they feel obliged to help. The results show that although students would be willing to help they may not posess the necessary knowledge to do so. They show that there is a demand for teaching focussed on these skills but that there is not necessarily a need for it taking into account the frequency of incidents encountered. A trial teaching programme will take place in July.

Teaching NeuroImages: Hyperintense acute reperfusion marker in ischemic stroke with transient symptoms
A 67-year-old man with 45-minute onset left hemiparesis underwent 3.0T gadolinium-enhanced MRI (reported iodine contrast allergy) with right frontoparietal small infarcts on diffusion-weighted images without vascular obstruction. No reperfusion therapy was given due to complete spontaneous symptoms regression. Twenty-four hours later, a second MRI showed right parietal CSF space enhancement on fluid-attenuated inversion recovery. Neither MRI nor CT had sign of hemorrhage (figure).

Evaluation of time to aneurysm treatment following subarachnoid hemorrhage: comparison of patients treated with clipping versus coiling
Conclusions Older patients with SAH and those treated at non-teaching hospitals were more likely to have delays to aneurysm clipping procedures. These associations were unique to open surgery as age and hospital teaching status did not affect time to coiling procedures.

Teaching Video NeuroImages: Foville syndrome
A 68-year-old man with a pontine telangiectasia on anticoagulation developed a left conjugate gaze palsy and right hemiparesis (figure; video on the Neurology® Web site at Neurology.org). Brain MRI showed acute pontine hemorrhage extending dorsally and to the left.

Teaching NeuroImages: Multicompartmental intracranial hemorrhage in a pediatric patient
A previously healthy 9-year-old girl presented with sudden-onset headache followed by confusion. In the emergency department, she was hypertensive, bradycardic, and no longer responsive to any stimuli. An emergent head CT scan demonstrated a left frontal intraparenchymal hematoma with intraventricular and subarachnoid extension as well as subfalcine herniation (figure). She underwent decompressive hemicraniectomy, and digital subtraction angiography confirmed a 7-mm left middle cerebral artery aneurysm (figure).

Development and Validation of a Grading Scale for Primary Pontine Hemorrhage Clinical Sciences
Background and Purpose—We aimed to develop and validate a grading scale for predicting 30-day mortality and 90-day functional outcome in patients with primary pontine hemorrhage (PPH).Methods—We retrospectively reviewed records of consecutive patients with first-ever pontine hemorrhage from 3 teaching hospitals between 2005 and 2012. Independent factors associated with 30-day mortality were identified by logistic regression to establish a risk stratification scale, named the new PPH score. For validation of the new PPH score, we prospectively recruited subjects from 10 units between December 2014 and November 2015. The...

Teaching NeuroImages: Superficial siderosis due to a dural cervical arteriovenous fistula
A 58-year-old man presented to our hospital with cerebellar ataxia, pyramidal signs, dysarthria, bilateral deafness, and cognitive impairment. These symptoms were consistent with superficial siderosis (SS) of the CNS, confirmed by MRI (figure A).1 Cerebral angiography showed a dural arteriovenous fistula perispinal and pontic with venous drainage in the left foramen C1-C2, fed by meningeal branches of the vertebral artery (figure, B). The fistulous point was clipped. SS of CNS is a rare disease resulting from hemosiderin deposition on the surface of the CNS and cranial nerves.2 At diagnosis, the etiology may...
