High-On Treatment Platelet Reactivity as a Risk Factor for Secondary Prevention after Coronary Stent Revascularization: A Landmark Analysis of the ARCTIC Study.
CONCLUSIONS: Detection of platelet hyper-reactivity by platelet function testing in patients undergoing coronary stenting with further therapeutic adjustment does not reduce ischemic recurrences after intervention. On-treatment platelet hyper-reactivity cannot be considered as a risk factor requiring intervention for secondary prevention after percutaneous coronary revascularization.

Stroke after Aortic Valve Surgery: Results from a Prospective Cohort.
CONCLUSIONS: Clinical stroke after AVR was more common than previously reported, more than double for this same cohort in the STS database, and silent cerebral infarctions were detected in over half of patients undergoing AVR. Clinical stroke complicating AVR is associated with increased length of stay and mortality.

Time Dependent Changes in Atherosclerotic Plaque Composition in Patients Undergoing Carotid Surgery.
CONCLUSIONS: In conclusion, over the past decade, atherosclerotic plaques harvested during carotid endarterectomy show a time dependent change in plaque composition characterized by a decrease in features currently believed to be causal for plaque instability. This seems to go hand in hand with improvements in risk factor management.

Predictors of Long-Term Recurrent Vascular Events after Ischemic Stroke at Young Age: The Italian Project on Stroke in Young Adults.
CONCLUSIONS: Among patients with IS aged 18 to 45 years, the long-term risk of recurrent thrombotic events is associated with modifiable, age-specific, risk factors. The IPSYS score may serve as a simple tool for risk estimation.

Impact of Incident Venous Thromboembolism on Risk of Arterial Thrombotic Diseases.
CONCLUSIONS: Our findings imply that women and young men with VTE have higher risk of arterial thrombotic disease than those without VTE. However, only 1% of the arterial thrombotic events in the population are attributed to VTE.

Inhospital Complications Associated with Catheter Ablation of Atrial Fibrillation in the United States between 2000-2010: Analysis of 93,801 Procedures.
CONCLUSIONS: The overall complication rate was 6.29% in patients undergoing AF ablation. There was a significant association between operator and hospital volume on adverse outcomes. This suggests a need for future research into identifying the safety measures in AF ablations and instituting appropriate interventions to improve overall AF ablation outcomes.

Early Dual versus Mono Antiplatelet Therapy for Acute Non-Cardioembolic Ischemic Stroke or Transient Ischemic Attack: An Updated Systematic Review and Meta-Analysis.
CONCLUSIONS: For patients with acute non-cardioembolic ischemic stroke or TIA, dual therapy was more effective than monotherapy in reducing risks of early recurrent stroke. The results of the CHANCE study are consistent with previous studies done in other parts of the world.

Homoarginine Levels are Regulated by L-Arginine:Glycine Amidinotransferase and Affect Stroke Outcome: Results from Human and Murine Studies.
CONCLUSIONS: Low homoarginine appears related to poor outcome after ischemic stroke. Further validation in future trials may lead to therapeutic adjustments of homoarginine metabolism that alleviate stroke and other vascular disorders.

Stroke or Transient Ischemic Attack in Patients with Transvenous Pacemaker or Defibrillator and Echocardiographically Detected Patent Foramen Ovale.
CONCLUSIONS: In patients with endocardial leads, the presence of a PFO on routine echocardiography is associated with a substantially increased risk of embolic stroke/TIA. This finding suggests a role of screening for PFOs in patients who require CIEDs, if detected, PFO closure, anticoagulation, or non-vascular lead placement may be considered.

Long-Term Effects of Secondary Prevention on Cognitive Function in Stroke Patients.
CONCLUSIONS: Appropriate vascular risk management was associated with a long-term reduced risk of cognitive impairment. Focus on optimal preventive drug therapy of vascular risk factors and management should be supported.
