Association of Race with Mortality and Cardiovascular Events in a Large Cohort of US Veterans.
CONCLUSIONS: -African-American veterans with normal eGFR have lower all-cause mortality and incidence of CHD, and similar incidence of ischemic stroke. These associations are in contrast with the higher mortality experienced by African-American individuals in the general US population. PMID: 26384521 [PubMed - as supplied by publisher]

How Does Cardiovascular Disease First Present in Women and Men? Incidence of 12 Cardiovascular Diseases in a Contemporary Cohort of 1,937,360 People.
CONCLUSIONS: -The majority of initial presentations of CVD are neither MI nor ischemic stroke, yet most primary prevention studies focus on these presentations. Sex has differing associations with different CVDs, with implications for risk prediction and management strategies. Clinical Trial Registration Information-www.clinicaltrials.gov. Identifier: NCT01164371. PMID: 26330414 [PubMed - as supplied by publisher]

Clinical Effectiveness of Statin Therapy after Ischemic Stroke: Primary Results from the Statin Therapeutic Area of the Patient-Centered Research into Outcomes Stroke Patients Prefer and Effectiveness Research (PROSPER) Study.
CONCLUSIONS: -In older ischemic stroke patients who were not taking statins at the time of admission, discharge statin therapy was associated with lower risk of MACE and nearly a month more home-time during the 2-year period post-hospitalization. PMID: 26246175 [PubMed - as supplied by publisher]

Recanalization Therapies in Acute Ischemic Stroke Patients: Impact of Prior Treatment with Novel Oral Anticoagulants on Bleeding Complications and Outcome - A Pilot Study.
CONCLUSIONS: -IVT/IAT in selected patients with ischemic stroke under NOAC treatment has a safety profile similar to both, IVT/IAT in patients on subtherapeutic VKA-treatment or in those without prior anticoagulation. However, further prospective studies are needed, including the impact of specific coagulation tests. PMID: 26232277 [PubMed - as supplied by publisher]

Extracranial Systemic Embolic Events in Patients with Nonvalvular Atrial Fibrillation: Incidence, Risk Factors, and Outcomes.
CONCLUSIONS: -SEE constituted 11.5% of clinically-recognized thromboembolic events in patients with AF and was associated with high morbidity and mortality. SEE mortality was comparable to that of ischemic stroke and varied by anatomic site. PMID: 26224811 [PubMed - as supplied by publisher]

Combination of an Immune Modulator Fingolimod with Alteplase in Acute Ischemic Stroke: A Pilot Trial.
CONCLUSIONS: -In this pilot study, combination therapy of fingolimod and alteplase was well tolerated, attenuated reperfusion injury and improved clinical outcomes in AIS patients. These findings need to be tested in further clinical trials. Clinical Trial Registration Information-www.clinicaltrials.gov. Identifier: NCT02002390. Impact of Diabetes on Hospitalization for Heart Failure, Cardiovascular Events, and Death: Outcomes at 4 Years from the REACH Registry.
CONCLUSIONS: -Diabetes increases substantially the risk of death, ischemic events and heart failure. Patients with both diabetes and heart failure are at particularly elevated risk of cardiovascular death, highlighting the need for additional therapies in this high-risk population. PMID: 26152709 [PubMed - as supplied by publisher]

Cerebral Microbleeds are Associated With an Increased Risk of Stroke: The Rotterdam Study.
CONCLUSIONS: -Microbleeds on MRI are associated with an increased risk of stroke in the general population. Our results strengthen the notion that microbleeds mark progression of cerebrovascular pathology and represent a precursor of stroke. PMID: 26137955 [PubMed - as supplied by publisher]

How to Stay Cool in the ICU? Endovascular vs. Surface Cooling.
Abstract Dozens of observational studies published over the past two decades have shown that fever in patients with acute neurologic injury, regardless of its cause, is independently linked to higher mortality, poor neurologic outcome, and increased length of stay in the intensive care unit (ICU) and hospital. This has been demonstrated for traumatic brain injury, acute ischemic stroke (AIS), subarachnoid haemorrhage, intracranial haemorrhage, and cardiac arrest (CA).(1,2) Therefore, therapeutic temperature management (TTM) is a key goal of care in all patients with acute brain injury. In most cases the goal is st...

Young Women With Acute Myocardial Infarction and the Post-Hospital Syndrome.
Abstract Our awareness and understanding of ischemic heart disease (IHD) in women has evolved substantially over the past two to three decades. The myth that heart attacks are "male diseases" was clearly dispelled and we know now that cardiovascular diseases (CVD), including heart disease and stroke, are common and represent the number one killer of women in the United States and worldwide.(1,2) In the Framingham Heart Study it was estimated that among women free of CVD at 50 years of age lifetime risk for developing CVD was 39.2%.(3) We have learned that clinically manifest IHD usually develops a decade later in ...

Restarting Anticoagulant Treatment After Intracranial Haemorrhage in Patients With Atrial Fibrillation and the Impact on Recurrent Stroke, Mortality and Bleeding: A Nationwide Cohort Study.
CONCLUSIONS: -Oral anticoagulant treatment was associated with a significant reduction in ischemic stroke/all-cause mortality rates, supporting oral anticoagulant treatment reintroduction post-intracranial haemorrhage as feasible. Future trials are encouraged to guide clinical practice in these patients. PMID: 26059010 [PubMed - as supplied by publisher]

Fructosamine and Glycated Albumin and the Risk of Cardiovascular Outcomes and Death.
CONCLUSIONS: -The acceptance of new measures of hyperglycemia is partly dependent on establishing their association with long-term outcomes. We found that fructosamine and glycated albumin were associated with vascular outcomes and mortality and that these associations were similar to those observed for HbA1c. Risk of Intracranial Hemorrhage from Statin Use in Asians: A Nationwide Cohort Study.
CONCLUSIONS: -Generally, no association was observed between cumulative statin use and risk of ICH among subjects without a prior history of stroke. An increased risk was identified among the non-hypertensive cohort, but this finding should be interpreted with caution. PMID: 25858194 [PubMed - as supplied by publisher]
