Wide Variation in Reported Rates of Stroke Across Cohorts of Patients with Atrial Fibrillation.
CONCLUSIONS: -There is substantial variation across cohorts in overall stroke rates and rates corresponding to CHA2DS2-VASc point scores. These variations can affect the point score threshold for recommending OAC in AF. The majority of cohorts did not observe stroke rates that would indicate a clear expected net clinical benefit for anticoagulating AF patients with CHA2DS2-VASc scores of 1 or 2. Prevention of Stroke with the Addition of Ezetimibe to Statin Therapy in Patients with Acute Coronary Syndrome in IMPROVE-IT.
Conclusions -The addition of ezetimibe to simvastatin in patients stabilized after ACS reduces the frequency of ischemic stroke, with a particularly large effect seen in patients with a prior stroke. CONCLUSIONS: -Stroke burden in China has increased over the last 30 years, and remains particularly high in rural areas. Prevention of Stroke with Ticagrelor in Patients with Prior Myocardial Infarction: Insights from PEGASUS-TIMI 54 (Prevention of Cardiovascular Events in Patients With Prior Heart Attack Using Ticagrelor Compared to Placebo on a Background of Aspirin-Thrombolysis in Myocardial Infarction 54).
CONCLUSIONS: High-risk patients with prior myocardial infarction are at risk for stroke, approximately one-third of which are fatal or lead to moderate-to-severe disability. The addition of ticagrelor 60 mg twice daily significantly reduced this risk without an excess of hemorrhagic stroke but with more major bleeding. In high-risk patients with coronary disease, more intensive antiplatelet therapy should be considered not only to reduce the risk of coronary events, but also of stroke. Relationship of Familial Hypercholesterolemia and High LDL Cholesterol to Ischemic Stroke: The Copenhagen General Population Study.
Conclusions -FH and high LDL cholesterol did not confer an increased risk of ischemic stroke. A positive association with ischemic stroke observed for some clinical FH criteria and high LDL cholesterol appears to be due by previous ischemic heart disease, rather than by high LDL cholesterol per se. Clinical Effectiveness of Statin Therapy after Ischemic Stroke: Primary Results from the Statin Therapeutic Area of the Patient-Centered Research into Outcomes Stroke Patients Prefer and Effectiveness Research (PROSPER) Study.

Stroke in Adults With Congenital Heart Disease: Incidence, Cumulative Risk and Predictors.
CONCLUSIONS: -Among ACHD-patients, one in eleven men and one in fifteen women suffered a stroke between age 18 and 64. Stroke-incidence was considerably higher than in the general population, especially at younger age. The most important predictors of ischemic stroke were heart failure, diabetes and recent myocardial infarction. Novel Drug Targets for Ischemic Stroke Identified Through Mendelian Randomization Analysis of the Blood Proteome.
CONCLUSIONS: Through a systematic MR screen of the circulating proteome, causal roles for five established and two novel biomarkers for ischemic stroke were identified. Side-effect profiles were characterized to help inform drug target prioritization. Clinical Application of a Novel Genetic Risk Score for Ischemic Stroke in Patients with Cardiometabolic Disease.
Conclusions: Across a broad spectrum of subjects with cardiometabolic disease, a 32-SNP GRS was a strong, independent predictor of ischemic stroke. In patients with atrial fibrillation but lower CHA2DS2-VASc scores, the GRS identified patients with risk comparable to those with higher CHA2DS2-VASc scores. Early Versus Delayed Non-Vitamin K Antagonist Oral Anticoagulant Therapy After Acute Ischemic Stroke in Atrial Fibrillation (TIMING): A Registry-Based Randomized Controlled Noninferiority Study
CONCLUSIONS: Early initiation was noninferior to delayed start of NOAC after acute ischemic stroke in patients with atrial fibrillation. Long-term Incidence of Ischemic Stroke Following Transient Ischemic Attack: A Nationwide Study During 2014-2020
Conclusions: Patients with first-time TIA had an ischemic stroke incidence of 6.1% during the 5-year follow-up period.
