Hepatitis C and recurrent treatment-resistant acute ischemic stroke.
Abstract Since the introduction of recombinant tissue plasminogen activator and thrombolysis, acute ischemic stroke has become a treatable disorder if the patient presents within the 4.5-hour time window. Typically, sporadic stroke is caused by atherosclerotic disease involving large or small cerebral arteries or secondary to a cardioembolic source often associated with atrial fibrillation. In the over-65-year age group, more rare causes of stroke, such as antiphospholipid syndromes, are unusual; such stroke etiologies are mostly seen in a younger age group (<55 years). Here we describe acute ischemic stroke in...

Utility of routine transesophageal echocardiography in patients with stroke or transient ischemic attack.
Abstract In several series of patients with presumed stroke, transesophageal echocardiography (TEE) has seemed advantageous over transthoracic echocardiography (TTE) in that it improves detection of cardiac sources and can lead to changes in the methods of treatment. We hypothesized that the real-world application of TEE in our institution produced results at variance from those previously reported. Our study of 219 patients, referred for TEE for an indication of stroke or transient ischemic attack, found few relevant findings that would have indicated a new diagnosis or required a change in treatment. These resul...

Usefulness of neutrophil-to-lymphocyte ratio as a predictor of functional outcome in patients with acute ischemic stroke after thrombolysis therapy
Proc (Bayl Univ Med Cent). 2021 Jun 29;34(6):664-667. doi: 10.1080/08998280.2021.1938471. eCollection 2021.ABSTRACTIn stroke patients, a high neutrophil-to-lymphocyte ratio (NLR) has been associated with poor functional outcome at 3 months, higher mortality, and a higher hemorrhagic transformation. We assessed the role of NLR in patients with acute ischemic stroke who received thrombolysis. The charts of 98 patients treated between 2015 and 2019 were retrospectively reviewed. The mean NLR was compared using an independent Student's t test. At 3 months, a good functional outcome was seen in 31 patients (32%) and a poor func...

Prevalence and impact of takotsubo syndrome in hospitalizations for acute ischemic stroke
In conclusion, the prevalence of TTS in AIS remained ∼20 times higher than in the general inpatient population and predisposed AIS patients to worse inpatient outcomes. Further studies are needed to evaluate the impact of TTS on long-term outcomes in AIS.PMID:35261441 | PMC:PMC8865315 | DOI:10.1080/08998280.2021.1995932
