Purpuric herpes zoster in patients in therapy with clopidogrel
Clopidogrel hydrogen sulfate is an adenosine diphosphate receptor antagonist belonging to the thienopyridine class. It is used for the prevention of vascular events in patients with atherothrombotic diseases manifested by recent myocardial infarction, ischemic stroke or peripheral arterial disease [1]. Clopidogrel provides greater reduction of the risk of recurrent ischemic events than acetylsalicylic acid (ASA) in patients with a medical history of coronary artery bypass surgery, diabetes mellitus and in those in therapy with statins [1].

Clinical Picture Ischaemic scalp ulceration and hair loss
A 46-year-old woman presented to our outpatient clinic in June, 2013, with frequent collapse, ischaemic scalp ulcerations, and hair loss (). She had undergone carotid endarterectomy in 2005, after a small right hemispheric stroke, and had no other medical history apart from hypercholesterolaemia. She had no previous dermatological or scalp problems. She took clopidogrel 75 mg, aspirin 80 mg, and simvastatin 40 mg daily.

ConclusionAs our pilot study, tissue kallikrein would be expected to prevent the long‐term in‐stent restenosis after stenting of the symptomatic middle cerebral artery dramatically.

Urgent Best Medical Therapy May Obviate the Need for Urgent Surgery in Patients With Symptomatic Carotid Stenosis Clinical Sciences
Conclusions— CEA can be performed in the subacute period without significantly increasing the operative risk. The urgent best medical treatment was associated with significant reduction in the risk of early NR in CEA patients. It seems that urgent aggressive best medical treatment may obviate the need for urgent CEA.

ConclusionsIn patients undergoing carotid stenting, a strategy using both a 600-mg clopidogrel load and a short-term reload with high-dose atorvastatin protects against early ischemic cerebral events. These results, obtained along with routine mechanical neuroprotection, provide new evidence of the optimization of drug therapy before percutaneous carotid intervention.
