Conclusion: This study demonstrated the success of a curriculum-style educational intervention using multimedia technology on improving knowledge and performance of cardiologists which can lead to enhanced management of CV risks and, thus, improved outcomes in patients with T2D and CVD.

The United States is in the midst of a historic demographic shift in its population that will have multiple societal impacts including healthcare issues. In 2043 it is predicted that the majority of the US population will be persons of color ("racial and ethnic minorities"). This new majority will be 53.4% of the nation by 2050. Of the 49 million uninsured in the US in 2011, 55% were persons of color who were only 33% of the population. Abstract 142: Primary Endpoint Results from a Community-Based Registry Evaluating the Use of a Blood-Based Age/Sex/Gene Expression Test in Patients Presenting with Symptoms Suggestive of Obstructive Coronary Artery Disease: the PRESET Registry (A Registry to Evaluate Patterns of Care Associated with the Use of Corus(R) CAD in Real World Clinical Care Settings) Session Title: Poster Session I
Conclusion: In this analysis of a community-based cardiovascular registry, the age/ex/gene expression test was adopted in clinical practice and associated with a statistically significant and clinically relevant effect on medical decision making in patients presenting with typical or atypical symptoms suggestive of obstructive CAD. The use of the ASGES test showed clinical utility in helping clinicians efficiently and safely rule out obstructive CAD as the cause of their patients’ symptoms, thereby minimizing potentially unnecessary referral of low risk patients to cardiology and further cardiac diagnostic testing.

Abstract 146: Clinical and Economic Consequences of Statin Intolerance in the U.S.: Results from an Integrated Health System Session Title: Poster Session I
Conclusion: While the majority of SI patients were on a statin, SI patients demonstrate a higher risk of some cardiovascular events; incur higher healthcare costs; and difficulty reaching LDL-C goals compared to patients without SI. Alternative treatment strategies are needed to better serve this at-risk patient population.

Abstract 328: Cardiovascular Disease Associated with Ability to Achieve Renal Transplant and Post-Transplant Adverse Events Session Title: Poster Session III
Conclusions: CVD is common in patients listed for renal transplant. CAD is independently associated with lower odds of receiving a transplant. CAD and rEF are independently associated with increased hazard of post-transplant death or graft failure. Future efforts should focus measures to optimize outcomes in patients with CVD awaiting transplant.

Conclusions: Hospital performance with scheduling primary care follow-up appointments improved significantly; however, only 1 in 4 patients had an appointment scheduled prior to discharge. Case study analysis of missed opportunities may help identify barriers and facilitators associated with access, availability, and awareness that can be addressed in future improvement cycles.

Abstract 263: Incidence of Recurrent Cardiovascular Events and Disease Burden Among High-Risk Patients with Hyperlipidemia Session Title: Poster Session II
Conclusions: CV event-related risk and long-term costs are significantly greater among high-risk patients with shorter time intervals between recurrent CV events. Underutilization of LLTs in these patients highlights the need for improving clinical management and treatment options for these patients.

Conclusion: This study represents the largest clinical experience of the safety of thrombolysis in patients on prior antiplatelet therapy. Despite a higher incidence of sICH, the absolute excess risk appears small (0.67%). These findings support current guideline recommendations regarding use of intravenous tPA in patients on antiplatelet therapy with careful weighing of potential risk and benefit.

Conclusion: A substantial percentage of patients deemed "too good" for IV tPA were unable to be discharged home. Factors such as advanced age and higher NIHSS should be considered in tPA decision-making to optimize outcomes. Large, multi-center prospective studies are underway to study the predictors of poor outcomes in this group.
