Left atrial appendage closure in a patient with left atrial appendage thrombus using a novel fish ball technique
We report on a 47-year-old (body-mass-index 24, 8), male patient who was scheduled for percutaneous left atrial appendage (LAA) closure at our hospital. In this patient permanent atrial fibrillation was known since 2010 with a CHA2DS2-Vasc-score of 3, because of arterial hypertension, diabetes mellitus type II and coronary heart disease). The Hasbled-score was 4, because of arterial hypertension, bleeding and stroke in his history and labile INR values under anticoagulation with warfarin therapy.

Secular changes in rates of coronary heart disease, fatal coronary heart disease, and out-of-hospital fatal coronary heart disease
In the Atherosclerosis Risk in Communities (ARIC) surveillance study, out-of-hospital fatal coronary heart disease (CHD) declined more slowly than total fatal CHD over 1987–2008 ; however, the upper age limit was 74 and temporal changes in risk factors and preventive therapies were not considered. To evaluate secular trends in CHD, fatal CHD, and out-of-hospital fatal CHD in a broader age range accounting for changes in risk factors and therapies, we used data from the ARIC cohort study , the Cardiovascular Health Study (CHS) , and the REasons for Geographic And Racial Differences in Stroke (REGARDS) study . ARIC and CHS...

Successful management of a twice complicated case by implantation of three CoreValve prostheses
An 80year old man, diabetic on oral medication and with permanent atrial fibrillation with a single chamber pacemaker, was referred to our hospital for severe symptomatic aortic valve stenosis. His trans-thoracic echocardiography confirmed severe aortic stenosis with a mean/peak gradient of 45/85mmHg, an estimated aortic valve area of 0.7cm2 and good systolic function with an ejection fraction of 50%. His past medical history was remarkable for a minor stroke without neurological impairment and moderate kidney dysfunction with creatinine of 2.3mg/dL. A cardiac catheterization revealed no obstructive coronary heart disease.

Shared and non-shared familial susceptibility of coronary heart disease, ischemic stroke, peripheral artery disease and aortic disease
Abstract: Background: Little is known about whether the four main manifestations of arterial vascular disease (coronary heart disease=CHD, ischemic stroke, peripheral artery disease=PAD, and aortic (i.e. atherosclerosis/aneurysm) disease=AD) share familial susceptibility. The aim of this nationwide study was to determine the familial risks of concordant (same disease in proband and exposed relative) and discordant (different disease in proband and exposed relative) cardiovascular disease (CVD).Methods: Data from the Swedish Multigeneration Register on individuals aged 0–76years were linked to Swedish Hospital Discharge R...

The change in the atrial fibrillation type as a prognosis marker in a community study: Long-term data from AFBAR (Atrial Fibrillation in the BARbanza) study
Abstract: Background: The aim is to describe the prognosis role of the change in the atrial fibrillation (AF) type in an unselected population of patients with AF currently attending primary care in a single health-service area in Galicia, north-western Spain. Methods: AFBAR is a cohort study that was carried out by 35 primary care providers in 2008. Participants were followed up for a mean of 2.8±0.7years. 798 patients with the diagnosis of AF who presented at their clinics during a three-month period were recruited. Primary endpoint was mortality or hospital admission. Results: 778 patients (413 male) were analyzed; mean ...
