The New Strategy in Infective Endocarditis: Early Surgery Based on Early Diagnosis: Are We Too Late, When Early Surgery is Best?
Abstract Patients with infective endocarditis (IE) have a poor prognosis due to complications including congestive heart failure, paravalvular abscess formation, central and peripheral embolism, and hemorrhagic stroke.(1) Patients with IE and congestive heart failure (CHF) have a mortality as high as 50%.(1) The poor prognosis seems not be improved during recent years.(2) In Sweden 7.603 patients with IE received a close follow-up with assessment of the 30-day mortality and 5-year follow-up including autopsy in all fatal events sampled from 1997 to 2007.(2) During the observation period no change of mortality was ...

Bariatric Surgery: "Roux"-minating on Endothelial Cell and HDL Function.
Abstract Obesity, defined as a body mass index ≥ 30 kg/m2, is pandemic. Though prevalence in the United States has plateaued at ~31% of adults (78 million), worldwide obesity rates continue to rise(1). Perhaps more concerning is the high level of childhood obesity: 16.9% in US and ~13% (and rising) in developing countries(2). Why the alarm? Longitudinal studies of overweight or obese individuals have identified a 20 fold increase in risk for developing diabetes mellitus type 2 (T2D) and 1.5 fold increase risk for cardiovascular disease (CVD), including myocardial infarction, stroke and heart failure(3, 4). As ob...

Impact of Diabetes on Hospitalization for Heart Failure, Cardiovascular Events, and Death: Outcomes at 4 Years from the REACH Registry.
CONCLUSIONS: -Diabetes increases substantially the risk of death, ischemic events and heart failure. Patients with both diabetes and heart failure are at particularly elevated risk of cardiovascular death, highlighting the need for additional therapies in this high-risk population. The Study of Cardiovascular Health Outcomes in the Era of Claims Data: The Cardiovascular Health Study.
CONCLUSIONS: -The use of diagnostic codes from claims data as clinical events, especially when restricted to primary diagnoses, leads to an underestimation of event rates. Additionally, claims-based events data represent a composite endpoint that includes the outcome of interest and selected (misclassified) non-event hospitalizations. SPS3 Evidence Supports Intensive Blood Pressure Control.
Abstract Interest in identifying the most appropriate targets for systolic blood pressure (SBP) lowering to reduce cardiovascular events in persons with hypertension has been piqued by the widely publicized results of the Systolic Blood Pressure Intervention Trial (SPRINT)(1,2). SPRINT found overwhelming benefit (25% reduction in the primary composite outcome of myocardial infarction (MI), acute coronary syndrome not resulting in MI, stroke, acute decompensated heart failure, or death from cardiovascular causes) and 27% reduction in all-cause mortality among participants randomized to a SBP target of < 120 mm H...

Epidemiology and Mechanisms of Uremia-Related Cardiovascular Disease.
Abstract Patients with chronic kidney disease and end-stage renal disease are at 5- to 10-fold higher risk for developing cardiovascular disease (CVD) than age-matched controls. Clinically, CVD in this population manifests as coronary artery disease, arrhythmias, stroke, or congestive heart failure. Beyond the traditional risk factors (eg, diabetes mellitus and hypertension), uremia-specific factors that arise from accumulating toxins also contribute to the pathogenesis of CVD. In this review, we summarize the literature on the epidemiology of both traditional and uremia-related CVD and focus on postulated mechani...

Drugs That May Cause or Exacerbate Heart Failure: A Scientific Statement From the American Heart Association.
rgery and Anesthesia; Council on Cardiovascular and Stroke Nursing; and Council on Quality of Care and Outcomes Research Abstract Heart failure is a common, costly, and debilitating syndrome that is associated with a highly complex drug regimen, a large number of comorbidities, and a large and often disparate number of healthcare providers. All of these factors conspire to increase the risk of heart failure exacerbation by direct myocardial toxicity, drug-drug interactions, or both. This scientific statement is designed to serve as a comprehensive and accessible source of drugs that may cause or exacerbate heart f...

Sodium Glucose Cotransporter 2 Inhibitors in the Treatment of Diabetes: Cardiovascular and Kidney Effects, Potential Mechanisms and Clinical Applications.
Abstract Sodium glucose co-transporter-2 (SGLT2) inhibitors, including empagliflozin, dapagliflozin and canagliflozin, are now widely approved anti-hyperglycemic therapies. Due to their unique glycosuric mechanism, SGLT2 inhibitors also reduce weight. Perhaps more importantly are osmotic diuretic and natriuretic effects contributing to plasma volume contraction, and decreases in systolic and diastolic blood pressures (BP) by 4-6/1-2 mmHg, respectively, which may underlie cardiovascular and kidney benefits. SGLT2 inhibition is also associated with an acute, dose-dependent reduction in eGFR by ~5 ml/min/1.73m(2) and...

Medical Therapy for Secondary Prevention and Long-Term Outcome in Patients with Myocardial Infarction with Non-Obstructive Coronary Artery (MINOCA) Disease.
Conclusions -The results indicate long-term beneficial effects on outcome in patients with MINOCA of treatment with statins and ACEI/ARBs, a trend toward a positive effect of beta-blocker treatment, and a neutral effect of DAPT. Properly powered randomized clinical trials to confirm these results are warranted. PMID: 28179398 [PubMed - as supplied by publisher]
