A Phase 2a dose ‐escalation study of the safety, tolerability, pharmacokinetics and haemodynamic effects of BMS‐986231 in hospitalized patients with heart failure with reduced ejection fraction
ConclusionsBMS‐986231 demonstrated a favourable safety and haemodynamic profile in patients hospitalized with advanced heart failure. Based on preclinical data and these study's findings, it is possible that the haemodynamic benefits may be mediated by inotropic and/or lusitropic as well as vasodilatory effects. The therapeutic potential of BMS‐986231 should be further assessed in patients with heart failure.

Implication of pulmonary hypertension in patients undergoing MitraClip therapy: results from the German Transcatheter Mitral Valve Interventions (TRAMI) Registry
ConclusionsDespite higher mortality in patients with elevated sPAP, these data suggest the safety, feasibility and benefit of MitraClip therapy even in advanced stages of disease. An early approach might prevent the progress of pulmonary hypertension and improve outcomes.

ConclusionsCardiac amyloid deposits are associated with severely reduced inotropic myocardial reserve and increased left and right ventricular filling pressures during exercise. Furthermore, CA subjects have severely reduced PAC, which may contribute to right heart failure and reduced exercise capacity.

Long ‐term outcomes for different surgical strategies to treat left ventricular outflow tract obstruction in hypertrophic cardiomyopathy
ConclusionSeptal myectomy is a safe procedure resulting in symptomatic improvement in the majority of patients. The annual incidence of non‐fatal disease‐related complications after surgical treatment of LVOTO is relatively high. Patients who underwent MV replacements had poorer outcomes with less symptomatic benefit in spite of a similar reduction in LVOT gradients.

Contribution of cardiac and extra ‐cardiac disease burden to risk of cardiovascular outcomes varies by ejection fraction in heart failure
ConclusionIn North American HF patients enrolled in the CHARM trials, the relative contribution of cardiac and extra‐cardiac disease burden to CV outcomes and death differed depending on LVEF. The high risk of events attributable to non‐cardiac disease burden may help explain why cardiac disease‐modifying medication proven to be efficacious in HFrEF patients has not proven beneficial in HFpEF.

Heart failure (HF) is a complex clinical syndrome with multiple interactions between the failing myocardium and cerebral (dys‐)functions. Bi‐directional feedback interactions between the heart and the brain are inherent in the pathophysiology of HF: (i) the impaired cardiac function affects cerebral structure and functional capacity, and (ii) neuronal signals impact on the cardiovascular continuum. Platelet activation is a preoperative risk factor for the development of thromboembolic complications in patients with continuous ‐flow left ventricular assist device
ConclusionsThrombotic events are associated with altered PAS values. Moreover, baseline elevated PAS values in patients who developed thrombotic events suggest patient‐specific tendency to post‐implant thromboembolic complications. Prospectively, systematic monitoring of PAS might guide the development of refined patient‐tailored antithrombotic strategies and the technological improvement of LVAD design.

Clinical correlates and outcome associated with changes in 6 ‐minute walking distance in patients with heart failure: findings from the BIOSTAT‐CHF study
ConclusionsThe 6 ‐minute walk test distance at baseline and a decline in walking distance were both associated with worse prognosis but did not improve the prognostic models. 6MWT distance was not modified by treatment up‐titration and its use for assessing the benefits of pharmacologic treatment up‐titration may be limited.

ConclusionsVICTORIA comprises a broadly generalizable high ‐risk population of three unique clinical strata of worsening chronic HFrEF despite very good HF therapy. VICTORIA will establish the role of vericiguat, a soluble guanylate cyclase stimulator, in HFrEF.
