Sleep Duration and Cardiovascular Disease Prevalence – The Debate Continues
We read with interest the study by Krittanawong et al “Sleep Duration and Cardiovascular Health in A Representative Community Population (from NHANES, 2005 to 2016)”.1 The authors studied 32,152 participants who responded to the sleep survey and found short sleep duration (9 hours) was also associated with higher prevalence of stroke, heart failure and coronary artery disease.

Comparison of Complications and In-Hospital Mortality in Takotsubo (Apical Ballooning/Stress) Cardiomyopathy versus Acute Myocardial Infarction
There are limited data on the incidence of complications and in-hospital outcomes, among patients with Takotsubo cardiomyopathy (TC), as compared to acute myocardial infarction (AMI). From 2007-2014, a retrospective cohort of TC was compared to AMI using the National Inpatient Sample database. Complications were classified as acute heart failure, ventricular arrhythmic, cardiac arrest, high-grade atrioventricular block, mechanical, vascular/access, pericardial, stroke and acute kidney injury. Temporal trends, clinical characteristics and in-hospital outcomes were compared.

Prognostic Implications of Significant Isolated Tricuspid Regurgitation in Patients With Atrial Fibrillation Without Left-Sided Heart Disease or Pulmonary Hypertension
The prognostic impact of isolated tricuspid regurgitation (TR) in patients with atrial fibrillation (AF) has not been investigated. The purpose of this study was to investigate the prognostic implications of significant isolated TR in AF patients without left-sided heart disease, pulmonary hypertension or primary structural abnormalities of the tricuspid valve.A total of 63 AF patients with moderate and severe TR were matched for age and gender to 116 AF patients without significant TR. Comparison of Frequency of Atrial Fibrillation in Blacks versus Whites and the Utilization of Race in a Novel Risk Score
Atrial fibrillation (AF) is the most prevalent sustained arrhythmia in the world, with over 30 million people affected as of 2010.1 The number of patients projected to develop AF will double over the next decade.2 AF is independently associated with an increased risk of stroke,3 heart failure (HF),4 and mortality.5 It is not surprising, thus, that AF assumes a sizeable share of health care expenditure and its magnitude is reaching epidemic proportions.6 Blacks despite having a higher prevalence of traditional risk factors for AF, such as HF, hypertension, and diabetes mellitus, have been shown to have a lower incidence of AF.

Relation between Thyroid Function and Mortality in patients with Chronic Heart Failure
Thyroid hormones have genomic and non-genomic effects on the cardiovascular system. They increase stroke volume, heart rate, and reduce systemic vascular resistance. (1) Overt thyroid disease (both hypothyroidism and hyperthyroidism) can cause heart failure. (2) Subclinical thyroid dysfunction is present when serum thyroid-stimulating hormone level (TSH) is outside the reference range, but free thyroxine (fT4), and total or free triiodothyronine level (T3) are normal. (3) In subclinical hyperthyroidism, TSH is low, and in subclinical hypothyroidism, TSH is raised.

Optimization of GRACE Risk Stratification by N-Terminal Pro-B-Type Natriuretic Peptide Combined with D-dimer in Patients with Non-ST-elevation Myocardial Infarction
We aimed to explore the utility of multiple biomarkers with GRACE risk stratification for non-ST-elevation myocardial infarction (NSTEMI). A total of 1357 patients diagnosed with NSTEMI were enrolled in this study at multiple medical centers in Tianjin, China. The outcomes were 1-year all-cause death and major adverse cardiac events (MACE: all-cause death, hospital admission for unstable angina, hospital admission for heart failure, non-fatal recurrent myocardial infarction, and stroke). C-index, NRI and IDI were calculated to verify that the biomarkers improve the predictive accuracy of the GRACE score.

Effect of Adenotonsillectomy on Cardiac function in Children Age 5-13 years with Obstructive Sleep Apnea
Obstructive sleep apnea (OSA) is associated with hypertension, stroke, heart failure, coronary artery disease and atrial fibrillation in adults1-5. Currently, there is conflicting evidence regarding the degree of improvement in cardiovascular outcomes among adults who are treated for OSA6, 7. Several studies have shown that continuous positive airway pressure (CPAP) therapy in adults partially reverses left ventricular (LV) and right ventricular (RV) dysfunction6, lowers blood pressure8, decreases pulmonary artery pressure9 and improves endothelial dysfunction10.

Impact of Age on Gender Difference in Long-term Outcome of Patients with Acute Myocardial Infarction (From J-MINUET)
This study focused on the association between age and gender difference in long-term outcome. We analyzed data from 3283 consecutive patients who were included in a prospective, nationwide, multicenter registry (J-MINUET) from 2012 to 2014. The primary endpoint was the major adverse cardiovascular event (MACE), which was defined as a composite of death, MI, stroke, heart failure, and revascularization for unstable angina during 3 years.

Prevalence, Incidence and Prognostic Implications of Left Bundle Branch Block in Patients with Chronic Coronary Syndromes (From the CLARIFY Registry)
Left Bundle Branch Block (LBBB) is a frequently encountered electrical abnormality in patients with chronic (more than 3 months after myocardial infarction, or evidence of coronary artery disease with ischemia) coronary syndromes (CCS), but its prognostic significance remains unclear. We aimed to describe the prevalence, incidence and five-year outcomes of LBBB in outpatients with CCS using the CLARIFY registry. Main outcome was a composite of CV death, MI or stroke. Secondary outcomes included all cause death, hospitalization for heart failure (HF) and permanent pacemaker implantation.

Effect of Lipoprotein (a) Levels on Long-term Cardiovascular Outcomes in Patients with Myocardial Infarction with Nonobstructive Coronary Arteries
The association between elevated lipoprotein(a) [Lp(a)] and poor outcomes in coronary artery disease (CAD) has been addressed for decades. However, little is known about the prognostic value of Lp(a) in patients with myocardial infarction with nonobstructive coronary arteries (MINOCA). A total of 1179 patients with MINOCA were enrolled and divided into low, medium, and high Lp(a) groups based on the cut-off value of 10 and 30mg/dL. The primary endpoint was major adverse cardiovascular events (MACE), a composite of all-cause death, nonfatal MI, nonfatal stroke, revascularization, and hospitalization for unstable angina or heart failure.
