Transapical aortic valve replacement in extreme-risk patients: outcome, risk factors and mid-term results ADULT CARDIAC
CONCLUSIONS TA-TAVR in extreme-risk patients carries a moderate risk of hospital mortality. Severe comorbidities and presence of residual paravalvular leakages affect the mid-term survival, whereas surviving patients have an acceptable quality of life without rehospitalizations for cardiac decompensation.

316 * ministernotomy versus conventional sternotomy for aortic valve replacement: propensity score analysis of 808 patients
Conclusions: Aortic valve replacement can be safely conducted through a partial ministernotomy. This approach is not associated with an increased rate of complications. Prospective studies with special emphasis on endpoints such as postoperative pain, duration of postoperative recovery, and quality of life during follow-up, are needed to further clarify the role of ministernotomy for aortic valve replacement.

Demographic profile, clinical characteristics and outcomes of patients undergoing coronary artery bypass grafting—retrospective analysis of 4,024 patients
Conclusion Compared to Western population, this cohort of patients who underwent coronary artery bypass grafting had higher incidence of diabetes mellitus and ischemic cardiomyopathy and low incidence of significant left main disease. Post-operative stroke rate was significantly lower compared to 1–2 % stroke rate reported in western population.

Use of Transaortic, Transapical, and Transcarotid Transcatheter Aortic Valve Replacement in Inoperable Patients.

Blood cardioplegia benefits only patients with a long cross-clamp time.
CONCLUSIONS: We did not find clear evidence of superiority of either type in the uncomplicated patient. When prolonged cross-clamp time or postoperative ventilation is expected, this study indicates that blood cardioplegia might be preferable. PMID: 30044166 [PubMed - as supplied by publisher]

Impact of occult renal impairment on early and late outcomes following coronary artery bypass grafting
CONCLUSIONS ORI was an independent risk factor for early and late death as well as cardiovascular events in patients undergoing CABG with normal SCr levels. A more accurate evaluation of renal function through a combination of SCr and estimated GFR is needed in patients with normal SCr levels.

Multimodal analgesia versus traditional opiate based analgesia after cardiac surgery, a randomized controlled trial
Background: To evaluate if an opiate sparing multimodal regimen of dexamethasone, gabapentin, ibuprofen and paracetamol had better analgesic effect, less side effects and was safe compared to a traditional morphine and paracetamol regimen after cardiac surgery. Preoperative atrial fibrillation and outcome in patients undergoing on-pump or off-pump coronary bypass surgery: lessons learned from the GOPCABE trial
CONCLUSIONS AF at admission is a significant risk factor for elderly patients undergoing coronary bypass grafting. However, this risk is not altered by performing bypass grafting off pump.
