Predictors of Long-Term Outcomes in Patients with Significant Myxomatous Mitral Regurgitation Undergoing Exercise Echocardiography.
CONCLUSIONS: In patients with ≥III+ myxomatous MR undergoing ExEc, lower %age-gender predicted METS, lower HRR, atrial fibrillation, lower LV ejection fraction and high resting RVSP predicted worse outcomes. PMID: 24396041 [PubMed - as supplied by publisher]

Impact of Incident Venous Thromboembolism on Risk of Arterial Thrombotic Diseases.
CONCLUSIONS: Our findings imply that women and young men with VTE have higher risk of arterial thrombotic disease than those without VTE. However, only 1% of the arterial thrombotic events in the population are attributed to VTE. PMID: 24270266 [PubMed - as supplied by publisher]

Using Benefit-Based Tailored Treatment to Improve the Use of Antihypertensive Medications.
CONCLUSIONS: We found that benefit-based tailored treatment was both more effective and required less antihypertensive medication than current guidelines based on treating to specific blood pressure goals. PMID: 24190955 [PubMed - as supplied by publisher]

Dyslipidemia, Coronary Artery Calcium, and Incident Atherosclerotic Cardiovascular Disease: Implications for Statin Therapy from the Multi-Ethnic Study of Atherosclerosis.
CONCLUSIONS: CAC may have the potential to help match statin therapy to absolute CVD risk. Across the spectrum of dyslipidemia, event rates similar to secondary prevention populations were observed for patients with CAC>=100. PMID: 24141324 [PubMed - as supplied by publisher]

Optimal Duration of Dual Antiplatelet Therapy after Drug-Eluting Stent Implantation: A Randomized Controlled Trial.
CONCLUSIONS: Among patients who were on 12-month dual antiplatelet therapy without complications, an additional 24 months of dual antiplatelet therapy versus aspirin alone did not reduce the risk of the composite end point of death from cardiac causes, myocardial infarction or stroke. Plasminogen Activator Inhibitor-1: A Novel Therapeutic Target for Hypertension?
Abstract Hypertension is the most common reversible risk factor for stroke, myocardial infarction, and heart failure, affecting greater than 60 million individuals in the United State alone and 1 billion globally(1). According to the World Health Organization, hypertension is now the leading cause of preventable death worldwide(2). Importantly, the prevalence and complications of hypertension increase significantly with age. However, despite the availability of many anti-hypertensive agents of distinct pharmacologic classes and evidence-based guideline recommendations for step-wise, multi-drug regimens, control of...

C-Reactive Protein, but not Low-Density Lipoprotein Cholesterol Levels, Associate with Coronary Atheroma Regression and Cardiovascular Events Following Maximally Intensive Statin Therapy.
CONCLUSIONS: Following 24 months of potent statin therapy, on-treatment CRP levels associated with MACE. Inflammation may be an important driver of residual cardiovascular risk in patients with coronary artery disease despite aggressive statin therapy. Causes of Death and Influencing Factors in Patients with Atrial Fibrillation: A Competing Risk Analysis from the Randomized Evaluation of Long-Term Anticoagulant Therapy Study.
CONCLUSIONS: The majority of deaths are not related to stroke in a contemporary anticoagulated AF population. These results emphasize the need to identify interventions beyond effective anticoagulation, in order to further reduce mortality in AF. Secular Trends in Cardiovascular Disease and its Risk Factors in Japanese: Half Century Data from the Hisayama Study (1961-2009).
CONCLUSIONS: Our findings suggest that in Japanese, the decreasing trends in the incidence of ischemic stroke have recently slowed down, and there has been no clear change in the incidence of acute myocardial infarction, probably because the benefits of hypertension control and smoking cessation have been negated by increasing metabolic risk factors. CONCLUSIONS: Among patients with type 2 diabetes and CAD in the BARI 2D trial, neither on-treatment nor propensity matched analysis supported an association of rosiglitazone treatment with an increase in major ischemic cardiovascular events. Elderly Patients with Acute Coronary Syndromes Managed Without Revascularization Insights into the Safety of Long-Term Dual Antiplatelet Therapy with Reduced-Dose Prasugrel vs. Standard-Dose Clopidogrel.
CONCLUSIONS: Older age is associated with substantially increased long-term cardiovascular risk and bleeding among medically managed ACS patients, with no differences in ischemic or bleeding outcomes with reduced-dose prasugrel vs. clopidogrel in elderly patients. No significant interaction among weight, pharmacodynamic response, and bleeding risk was observed between reduced-dose prasugrel vs. clopidogrel in elderly patients. Mitochondrial DNA Damage, Oxidative Stress and Atherosclerosis: Where There is Smoke There is not Always Fire.
Abstract Atherosclerosis is an insidious disease which can remain undetected for decades before manifesting, sometimes lethally, in the form of a stroke or myocardial infarction. With over 900,000 Americans experiencing a heart attack annually(1), there is a strong need to develop better treatments or preventative measures, but these will necessitate a better understanding of the underlying process. Most evidence suggests that atherosclerosis is initiated by chronic endothelial injury in response to the accumulation of low-density lipoprotein (LDL) and oxidized-LDL in the vessel wall, which stimulates an inflammat...
