Defining remission in asthma care
Treatments for asthma continue to evolve and with the development of biologics, we are now entering an arena where some patients appear to have their asthma symptoms disappear entirely. This dramatic improvement in a chronic disease is reminiscent of remission in other chronic illnesses, such as rheumatoid arthritis, but until now there has been no accepted definition for remission in asthma. In this issue of Annals, a workgroup that involved asthma experts from North America have put together a list of 6 criteria that they have defined as indicating remission of asthma.

Post thymectomy: an example of immune dysregulation?
The thymus is essential for creating the T cell repertoire while maintaining self-tolerance. Systemic autoimmune diseases (AD's) such as Rheumatoid Arthritis, Systemic Lupus Erythematous (SLE), and Sjogren's Disease, may unveil many years after thymectomy in patients with Myasthenia Gravis. Immunoglobulin G4-Related Disease (IgG4-RD) characterized by dysregulation of immune system affecting a single or multiple organs, has not been described post thymectomy.

A case of Perioperative Anaphylaxis Presenting as Hereditary Alpha Tryptasemia
A 64-year-old Caucasian male with a past medical history of hypertension, anxiety, arthritis, depression, and hyperlipidemia presented to the operating room (OR) for a planned shoulder graft surgery. He has had multiple prior surgeries without complications. Prior to the procedure he received a local nerve block with Lidocaine and Bupivacaine in the post anesthesia care unit (PACU). Afterwards he was transported to the OR where he received general anesthesia that included the following: Propofol, Cefazolin, Succinylcholine, Midazolam, Rocuronium, Fentanyl, and an inhaled anesthetic.

M155 autoinflammatory disorder, immunodeficiency, or both?
Autoinflammatory disorders result from inborn errors causing innate immune system dysregulation. Clinical features include recurrent fever, rash, arthritis, and lymphadenopathy in the absence of infection. This can be contrasted with primary immunodeficiency, wherein a lack of functional immune response leads to recurrent infections. Nuclear factor- κ B essential modulator (NEMO) deficiency causes a spectrum of disease which can manifest with features of both autoinflammation and immunodeficiency.

A 6-Step Rapid Desensitization Protocol to Hydroxychloroquine
Hydroxychloroquine, a 4-amino-quinolone antimalarial medication, is widely used for the treatment of systemic lupus erythematosus (SLE), discoid lupus erythematosus (DLE), rheumatoid arthritis and related inflammatory conditions. Cutaneous adverse reactions typically develop within 2-33 days and manifest as type IV reactions typically as a maculopapular exanthem (MPE)1. Type 1 reactions resulting in urticaria and anaphylaxis are less commonly reported in the literature2. It is estimated that 10% of patients on hydroxychloroquine will develop a hypersensitivity reaction resulting in its discontinuation3.

M501 bamboo nodules in a rheumatoid patient, response to inhaled corticosteroids
Bamboo nodules are a submucosal nodule of the vocal cords. They are a rare association with rheumatoid arthritis, typically seen with rheumatoid nodules in other parts of the body like elbows and knees. Traditional treatment includes supportive measures, steroid injections of the nodules or surgical resection. In this case, we have a seronegative rheumatoid arthritis patient without systemic nodules that presented with bamboo nodules responding to inhaled corticosteroids.

Hypereosinophilia with Angioimmunoblastic T cell Lymphoma 1
A 58-year-old woman was evaluated for persistent eosinophilia. She had joint pain with effusions for 14 years and was treated for presumptive seronegative rheumatoid arthritis with prednisone and multiple disease-modifying antirheumatic drugs with minimal improvement (timeline in Figure 1). For 2.5 years prior to evaluation, she developed a persistent pruritic rash, chronic cough, and a 20 pound weight loss. She had no atopic history and was not taking any herbal supplements. She did travel to Mexico and the US Virgin Islands within the past 2 years.
