Clinical Characteristics and Outcomes of Chagas Disease in the United States: A Multicenter Retrospective Analysis
Am J Trop Med Hyg. 2023 Sep 11:tpmd230361. doi: 10.4269/ajtmh.23-0361. Online ahead of print.ABSTRACTChagas disease affects approximately 300,000 patients in the United States. We evaluated a multicenter U.S.-based network to obtain clinical characteristics and outcomes of chronic Chagas disease by disease forms. This was a U.S.-based, multicenter, population-based, retrospective cohort study. We queried TriNetX, a global research network, to identify patients with dual-positive IgG serology for Trypanosoma cruzi. We captured outcomes of interest for up to 5 years. We found 429 patients with evidence of dual-positive T. cr...

Multiple Strokes in a Latin American Patient
We present a case of a recent immigrant from El Salvador without past medical history who presented to our hospital with symptoms concerning for acute stroke. Brain magnetic resonance imaging (MRI) with gadolinium confirmed an acute stroke along with multiple prior infarcts involving different vascular beds. Head magnetic resonance arteriogram did not reveal any occlusions/stenosis or aneurysmal changes. His subsequent extensive evaluation included an electrocardiogram (ECG) that revealed bifascicular block and echocardiography that suggested an apical aneurysm, but images were limited to assess. To further assess the like...

Simvastatin improves cardiac function through Notch1 activation in BALB/c mice with chronic Chagas cardiomyopathy.
Conclusions and Implications: Simvastatin, probably acting through the Notch1 pathway, decreases inflammation, improving cardiac function in chronically T. cruzi-infected mice. PMID: 32393497 [PubMed - as supplied by publisher]

PiB-PET detects transthyretin-related cerebral amyloid angiopathy
Transthyretin (TTR) is an amyloidogenic protein synthesized primarily (>95%) by the liver and, to a lesser extent, by the choroid plexuses and retinal pigment epithelium. Hereditary TTR amyloidosis (ATTR) is a multisystem disorder that may manifest with 3 main clinical phenotypes: familial amyloid polyneuropathy (FAP), familial amyloid cardiomyopathy, and familial leptomeningeal amyloidosis.1 TTR-FAP is the most common presentation. More than 130 autosomal dominant mutations cause ATTR, with the Val30Met mutation being the most frequent variant worldwide: prevalence of up to 1:1,000 in endemic regions of Portugal, Swede...
