One-Year Clinical Outcomes With SAPIEN 3 Transcatheter Aortic Valve Replacement in High-Risk and Inoperable Patients With Severe Aortic Stenosis.
CONCLUSIONS: In this large, adjudicated registry of SAPIEN 3 HR and inoperable patients, the very low rates of important complications resulted in a strikingly low mortality rate at 1 year. Between 30 and 365 days, the incidence of moderate paravalvular aortic regurgitation did not increase, and no association between mild paravalvular leak and 1-year mortality was observed, although a small increase in disabling stroke occurred. These results, which likely reflect device iteration and procedural evolution, support the use of transcatheter aortic valve replacement as the preferred therapy in HR and inoperable patients with...

Clinical Profile and Consequences of Atrial Fibrillation in Hypertrophic Cardiomyopathy.
Conclusions -Transient symptomatic episodes of AF, although relatively uncommon in HCM, are unpredictable in frequency and timing, amenable to effective contemporary treatments, and infrequently progress to permanent AF. AF is not a major contributor to heart failure morbidity, nor a cause of arrhythmic sudden death, and when treated is associated with low-disease-related mortality, no different than for patients without AF. AF is an uncommon primary cause of death in HCM virtually limited to embolic stroke, supporting a low threshold for initiating anticoagulation therapy. Predictors of Long Term Outcomes in Symptomatic Hypertrophic Obstructive Cardiomyopathy Patients Undergoing Surgical Relief of Left Ventricular Outflow Tract Obstruction.
CONCLUSIONS: Symptomatic adult HCM patients undergoing surgery for relief of LVOT obstruction have low event rate during long-term follow up; with worse outcomes predicted by increasing age and presence of residual AF during follow up. PMID: 23770748 [PubMed - as supplied by publisher]

Predictors of Long-Term Outcomes in Patients with Significant Myxomatous Mitral Regurgitation Undergoing Exercise Echocardiography.
CONCLUSIONS: In patients with ≥III+ myxomatous MR undergoing ExEc, lower %age-gender predicted METS, lower HRR, atrial fibrillation, lower LV ejection fraction and high resting RVSP predicted worse outcomes. PMID: 24396041 [PubMed - as supplied by publisher]

Systolic and Diastolic Mechanics in Stress Cardiomyopathy.
CONCLUSIONS: SCM and LAD MI show severe diastolic dysfunction: at similar LV volumes, their diastolic pressures are more than twice as high as control; systolic dysfunction is equally reduced in SCM and LAD MI. Despite a completely different pathophysiology, as far as systolic and diastolic function, SCM is indistinguishable from acute LAD territory MI. PMID: 24503950 [PubMed - as supplied by publisher]

Preventing Cardiovascular Complications of Acute Infection: A Missed Opportunity?
Abstract "...This is an experimental verification of this medical concept, relatively new, suggesting that infections merit an important place in the etiology of human atheromatous arteritis..." -Gilbert A and Lion G, Comptes Rendus Hebdomadairesdes Séances et Mémoires de la Société de Biologie. Published 1889 The connections between infection and cardiovascular disease have been postulated for at least 125 years, when Gilbert and Lion observed that acute infection with the typhoid bacillus resulted in atherosclerotic changes to the rabbit aorta. In the last few decades, much work has investigated the role for...

Reversible De Novo Left Ventricular Trabeculations in Pregnant Women: Implications for the Diagnosis of Left Ventricular Non-Compaction in Low Risk Populations.
CONCLUSIONS: -Pregnancy induces de novo LV trabeculations in a significant proportion of women. The results suggest that left ventricular trabeculations occur in response to increased LV loading conditions or other physiologic responses to pregnancy and are not specific for LVNC. These factors should be considered when assessing individuals with LV trabeculations outside the context of symptoms of heart failure or familial cardiomyopathy. Genotype and Lifetime Burden of Disease in Hypertrophic Cardiomyopathy: Insights from the Sarcomeric Human Cardiomyopathy Registry (SHaRe).
Conclusions: The cumulative burden of HCM is substantial and dominated by heart failure and atrial fibrillation occurring many years following diagnosis. Young age of diagnosis and the presence of a sarcomere mutation are powerful predictors of adverse outcomes. These findings highlight the need for close surveillance throughout life, and the need to develop disease-modifying therapies. PMID: 30297972 [PubMed - in process]
