A Modified Technique for the Treatment of Severe Adolescent Hallux Valgus: A Modification of the First Metatarsal Double Osteotomy
Adolescent hallux valgus is a deformity of childhood that is often difficult to treat conservatively because of its progressive nature and surgically because of its high risk of complications. Despite numerous procedures described, there is still little consensus on the best technique available for deformity correction. One of the standard procedures to address severe adult hallux valgus was described by Peterson and Newman in 1993. This procedure involved an excision of the medial eminence, an extra-articular distal closing wedge osteotomy to correct the hallux valgus, and a proximal transverse opening wedge osteotomy to ...

Replacing the Fusion: Conversion of an Ankle Arthrodesis to a Total Ankle Arthroplasty
A painful ankle arthrodesis, whether from malunion, nonunion, or the development of adjacent hindfoot arthritis, can be a difficult problem for the patient and treating surgeon. Several surgical options are available, however, conversion of the arthrodesis to a total ankle replacement offers the potential for restoration of some degree of ankle motion, reduced pain, and improved overall patient function. This concept of an ankle arthrodesis takedown to total ankle replacement and the surgical technique are reviewed here in detail, along with relevant complications, available outcomes data, and the future of the technique. ...

Treatment of the Acute and Chronic Infected Total Ankle Arthroplasty
Total ankle arthroplasty (TAA) has been used with increasing frequency for the treatment of end-stage ankle arthritis. The rate of periprosthetic joint infection for TAA varies in the literature. When it occurs, it can have devastating consequences. Despite the large amount of literature published on the workup, treatment, and outcomes of periprosthetic joint infection in other areas of the body, there is a paucity of studies when it comes to the ankle. The results of surgical treatment of infected TAA are mixed and complication rates are high. The experience gained by the hip and knee surgeons, shoulder and elbow surgeons...

The Painful TAR
Total ankle implants have evolved consistently over the past decade, and with improvement in design, the frequency of total ankle replacement (TAR) use has increased. Implant survival, ease, and accuracy of implantation have all been shown to improve with second and third generation TAR designs, but even with improvements, complications remain a concern. One such complication is residual pain following an ankle replacement. The etiology of a painful TAR is multifactorial and includes infection, soft tissue impingement, neuritis, heterotopic ossification, periarticular arthritis, and aseptic loosening. Careful surgical tech...

Revision Total Ankle Replacement in the Setting of Significant Bone Loss
The use of total ankle arthroplasty (TAA) for the treatment of end-stage ankle arthritis is on the rise. Aseptic loosening and subsidence represent the most common complications leading to failure following TAA and can be associated with a significant amount of bone loss. Revision TAA has become a more viable treatment option but the management of bone loss in the tibia and talus remains a challenge. There is a paucity of literature regarding the success of revision TAA in the setting of surrounding bone loss. Published outcomes are mixed and complications can be quite serious. More recent implant designs, particularly of ...

Total Ankle Replacement With Advanced Varus and Valgus Deformities
Severe deformity in patients with end-stage ankle arthritis has long been considered a relative contraindication for total ankle replacement. However, advancements in surgical techniques and a better understanding of the importance of correcting foot deformity in conjunction with ankle deformity have led to satisfactory outcomes in this group of patients. Management of these deformities is contingent upon identifying all sources of deformities in the limb both proximal and distal, addressing the deformity in a systematic manner, achieving ligamentous stability at the ankle, and assuring dynamic stability with proper soft t...

Arthroscopic-assisted Tibiotalocalcaneal Intramedullary Nail Arthrodesis With Reamer-Irrigator-Aspirator Bone Grafting
Severe hindfoot arthritis often requires a multilevel fusion which can be performed with a variety of techniques. The operating surgeon must take several elements into account when choosing a surgical technique as patients requiring a hindfoot fusion has often had multiple surgeries or have compromised soft tissue envelopes. Regardless of the technique used, meticulous joint preparation is paramount and often compliments the means of internal fixation. Beyond joint preparation and internal fixation, the surgeon must also decide if bone grafting is necessary and if so how to obtain the sufficient amount. Given the aforement...

Large Intratendinous Peroneus Tertius Ganglion Excised Via a 2-incision Technique
We present the case of an intratendinous ganglion within peroneus tertius requiring surgical excision. We discuss the anatomy and function of peroneus tertius and the existing literature regarding intratendinous ganglion treatment. Level of Evidence: Diagnostic Level V. Paratendinous Scraping and Excision of Plantaris for Achilles Tendinopathy
We report on the results of a prospective case series involving 2 cohorts of patients between February 2011 and February 2015 undergoing this combined technique. One cohort included recreational athletes (group A), the second included patients who undertook minimal recreational activity (group B). In group A there were 19 patients (25 tendons). The mean age at surgery was 44 years (range, 20 to 55 y). The mean follow-up was 21 months (range, 7 to 38 mo). Patients were satisfied with the results in 21 of the 25 tendons (84%). The mean Victorian Institute of Sports Assessment-Achilles improved from 51 to 90 following sur...

Reconstruction of Chronic Ankle Instability With Hamstring Autograft
There are varied techniques in the literature for operative treatment of chronic ankle instability. Historically, ankle instability has been treated through direct repair, using imbrication of attenuated tissues. However, direct repair often is not a suitable option for patients with chronic instability that has developed as a result of generalized ligamentous laxity, failed previous stabilization, or paucity of competent local tissue. For these patients, reconstruction of the lateral ligaments is indicated and has been performed using various types of grafts, including semitendinosus grafts, gracilis grafts, peroneal graf...

Hamstring Reconstruction for Chronic Achilles Pathology
There are many accepted treatment options for chronic tears of the Achilles tendon. In cases involving a tendon gap of over 5 cm after debridement and removal of unhealthy tissue, a primary tendon reconstruction is recommended, as there is not enough healthy tissue to perform a direct repair. Various tendons have been used, including the peroneal tendons, the Flexor Hallucis Longus (FHL), as well as allografts including use of the Achilles, FHL, flexor digitorum longus, and semitendinosus tendons. We propose the use of a hamstring autograft, using both gracilis and semitendinosus. In addition, if the patient is young and...
