End-of-life care
Pract Neurol. 2024 Mar 14:pn-2023-004075. doi: 10.1136/pn-2023-004075. Online ahead of print.ABSTRACTMy 96-year-old aunt taught me so much about end-of-life care. As doctors dealing with long-term neurological conditions, our relationships with patients over many years should enable us to be much more involved in helping them make decisions about the ceilings of treatment.PMID:38485227 | DOI:10.1136/pn-2023-004075 (Source: Practical Neurology)

Kennedy's disease
Pract Neurol. 2024 Mar 14:pn-2023-004041. doi: 10.1136/pn-2023-004041. Online ahead of print.ABSTRACTA 57-year-old man developed worsening early morning headaches, muscle cramps and falls over 12 months. He had widespread fasciculation and was diagnosed with motor neurone disease, and treated with nocturnal hypoventilation. Based on this diagnosis, he made significant personal and financial decisions including retiring and selling his house. He subsequently developed a lump in his right breast and was found to have gynaecomastia. This triggered genetic testing for Kennedy's disease leading to the correct diagnosis. This ca...

Hepatic encephalopathy: a neurologist's perspective
Pract Neurol. 2024 Mar 7:pn-2023-003802. doi: 10.1136/pn-2023-003802. Online ahead of print.ABSTRACTLiver disease is increasingly common, estimated to affect over 25% of the world's population. Failure of the liver to maintain a normal metabolic milieu leads to impaired brain function (hepatic encephalopathy), and conditions that cause liver disease can themselves predispose to neurological disease. As neurologists' involvement with the acute take increases, it is important that we are familiar with the neurological complications of liver disease, their investigation and management, and to know which other neurological dis...

Dopa-responsive dystonia and paroxysmal dystonic attacks associated with < em > ATP1A3 < /em > gene variant
Pract Neurol. 2024 Mar 7:pn-2023-004045. doi: 10.1136/pn-2023-004045. Online ahead of print.ABSTRACTAn 18-year-old man had episodes of severe generalised dystonia, from aged 7 months and becoming progressively more frequent. He also had gradually developed interictal limb dystonia. He was initially diagnosed with paroxysmal kinesigenic dyskinesia but he did not improve with several medications. A levodopa trial led to levodopa-induced dyskinetic movements. However, a lower titration of 25 mg of levodopa two times per day substantially improved his motor features and quality of life. Laboratory investigations and MR scans o...
