[Immuno-oncological therapeutic options in high-risk non-muscle invasive bladder cancer].
Abstract High-risk nonmuscle invasive bladder cancer (HR NMIBC) is an immunological malignancy. The standard therapy for HR NMIBC is based on transurethral bladder tumor resection with adjuvant Bacillus Calmette Guérin (BCG) instillation therapy. To prevent progression in case of BCG-refractory disease, early radical cystectomy is considered the therapy of choice according to the German S3 guidelines. With the advent of checkpoint inhibitors for the treatment of metastatic urological malignancies, a novel option for bladder preservation has been introduced for the treatment of HR NMIBC. The c...

[The long journey from palpation to biopsy : The history of diagnosing prostate cancer].
Abstract Though it is thousands of years old, prostate cancer (PCa) has only been known for 200 years. Until about 50 years ago, the diagnosis could only be made by digital rectal examination. Although the first prostate biopsies were already implemented in the beginning of the last century, it only obtained importance with the introduction of prostate-specific antigen (PSA) testing in the early screening for PCa and the pathologist, thus, became an essential partner for the urologist. In more recent years, specific MRI has significantly increased the accuracy of prostate biopsies. Thus, the question arises w...

Abstract On 1 March 2020, the amendments to the German Protection Against Infection Act that were introduced by the act to protect against measles and strengthen vaccination prevention (Measles Protection Act) entered into force. The reason for the changes is that the number of individuals with measles has significantly increased in recent years. To protect public health, the Measles Protection Act has implemented regulations requiring that persons in certain institutions must either have adequate protection against measles or have immunity to measles. In this article the current legal situation with regard to hea...

Techniques of vascular approaches (arteries and veins) are discussed and tricks for vascular repair are explained. Generally, vascular injuries during urologic surgery are rare. However, hemorrhage due to vascular injury is a common cause of critical morbidity and mortality in the perioperative period. Susceptibility to vascular complications such as oncological debulking and revision surgery increase risk for damage. As vascular injuries range...

[Targeted molecular therapy and immunotherapy for prostate cancer].
Grünwald V Abstract For decades, the treatment of advanced prostate cancer was mainly based on the manipulation of the androgen receptor-controlled proliferation pathway. Chemotherapy only played an additional important role with the advent of taxanes. The progress in translational research in recent years has led to innovations in the therapeutic environment. With the decoding of the homologous repair deficiency (HRD) machinery and its ability to be influenced by PARP inhibitors, targeted therapies moved into the therapeutic focus for selected patients. The first positive phase III study for PARP inhib...

In the following article, we summarize the practice-relevant innovations in systemic therapy in the guidelines on prostate cancer, transitional cell carcinoma of the bladder and renal cell carcinoma. [German guideline on undescended testis-what is relevant in daily routine?]
Abstract With an incidence of 0.7-3% in male infants, undescended testicles is one of the most common congenital anomalies. In the first 6 months of life, the testicles may spontaneously descend in up to 70% of individuals. If the testicle is not in a scrotal position afterwards, fertility can gradually be reduced and the risk of a testicular tumor increases. Therefore, the current German guideline for undescended testis recommends that therapy should be take place between 6 and 12 months of life. After extensive information on the advantages and disadvantages, hormone therapy with the aim of a...

A rational base-line work-up of our patients provides the necessary information for obtaining the diagnosis and only needs to be expanded in individual cases. [Combination therapy for metastatic hormone-sensitive prostate cancer: What? When? For whom?]
Abstract Novel combination therapies are currently the standard systemic treatment for metastatic hormone-sensitive prostate cancer. As a result, the overall survival of patients can be extended by approximately 1.5 years. The taxane docetaxel, the CYP17 inhibitor abiraterone and the second generation antiandrogen apalutamide can currently be used as a combination partner for classic androgen deprivation. While the de novo synthesis of testosterone is the rationale for abiraterone or apalutamide combination, taxanes offer a promising approach in the presence of primarily androgen-independent pr...

[Vesicovaginal fistulas: diagnosis and surgical management].
Abstract Vesicovaginal fistulas (VVF) represent a detrimental condition causing psychological, physical and social strain on patients. In developed countries they are predominantly the result of pelvic surgery or radiation therapy, whilst obstetric VVF are common in developing countries due to prolonged and complicated births. The majority of VVF require surgical therapy, thus a comprehensive diagnostic workup is needed. Depending on diagnostic characteristics fistula repair can be performed by a transvaginal, transabdominal or minimally invasive approach. Timing of surgery, appropriate interposition of vasculariz...

[Complementary medicine in uro-oncology].
r J Abstract Modern concepts such as patient empowerment stand for the selection of appropriate concomitant treatment procedures from a scientific point of view and for placing these in the hands of the patient, thereby demanding and encouraging personal responsibility. It is important to distinguish between the terms complementary and/or alternative medicine. If we look at the motivation of patients to use complementary and/or alternative procedures, this is divided into the desire to do something directly against the tumor and on the other hand to strengthen the body's own resources, support the immune system, a...

Abstract Vesicovaginal fistulas are a rare problem in the western world but are frequent occurrences in developing countries. In Germany the most frequent cause is hysterectomy. Vesicovaginal fistulas can be treated by the transvaginal or transabdominal approach depending on the characteristics of the fistula and the patient. The incidence and complexity of urorectal fistulas increase with the number of cumulative sequences of prostate cancer treatment. Overall there is no clear consensus about the optimal surgical approach route. The surgical treatment of both vesicovaginal and urorectal fistulas is associat...

Abstract Anatomy and pathophysiology of the prostate have gained increasing attention of anatomists and surgeons at the beginning of the 19th century. It was only around 1900 that French and German authors discussed staging of clinical benign prostatic hyperplasia (BPH) in order to group therapy. From 1970 to the 1990s, staging of the clinical course of BPH was associated with the name of Carl-Erich Alken, a leading figure within the German urological society at that time, although Alken never researched or focused on disease staging. He only presented the three traditional clinical stages originally des...

Abstract The surgical treatment of Peyronie's disease involves a multistage procedure with increasing invasiveness. In addition to precise knowledge about the existing surgical treatment procedures, the indication and the informed consent process of the patient is extremely important. The dissatisfaction with the surgical results for many patients is due to false expectations and positivistic representations. Abstract As useful and necessary as radiotherapy may be, it is associated with irreversible long-term adverse effects including loss of bladder function. In the majority of cases the small bowel, sigmoid rectum and ureter are also affected. The extent of injury depends on the irradiated area, the applied dose and the quality of application. Three factors are essential when choosing a urinary diversion: the length of functional ureter, the position of the bowel outside of the irradiated area and the type of diversion. A continent diversion is best performed through a ascending-transverse or transverse-des...
