[Quality instead of quantity improves medical education].
JP Abstract Currently, continuing medical evaluation takes place in Germany-but not nationally or regularly, and without national standards. Therefore, comparisons between different clinics and trainers can currently not be drawn. Survey modes, such as those that have existed in Switzerland for decades, could provide a constructive basis in the assessment of the current continuing medical education of urology residents and subsequently facilitate the discussion on improvements. This requires constructive cooperation of all involved, without attributing the responsibility only to the educators. Models of rewa...

[Urological infections and antibiotic management in geriatric patients].
TRW Abstract Both the demographic shift and progress in medicine are resulting in an increasingly longer life expectancy. It is presumed that a mean age of 90 years will be achieved within the next decade in many countries. Thus, geriatric medicine, which is committed to the specific needs of older, often frail and frequently comorbid patients, is becoming increasingly more important. The prevalence of infections of the genitourinary tract increases with age, simultaneously, a critical and conscious use of antibiotics is required in terms of antimicrobial treatment. The intention of the present review is...

[Compulsory organ donation-a controversial topic]. Urologe A. 2019 Jun 28;: Authors: Hakenberg OW Abstract The number of organ donations in Germany has continuously fallen in recent years. The proposed Transplantation Act stipulates that every German citizen is considered an organ donor if he/she has not objected during his/her lifetime. This proposal has received wide approval, but also much criticism. The German Society for Urology, the German Society for Surgery, the German Society for Nephrology and the German Transplantation Society have all spoken out in press releases in favor of the intro...

[Affiliated urologists-what motivates us].
Abstract The idea of affiliated urologists is a well-established and functional intersectoral patient care concept. Due to policy changes, there has been a steady decline in the number of practicing affiliated urologists which is putting this organisational structure at risk. Patients benefit from this organisational structure as they only have one treating urologist and their treatment plan is not dependent on different treatment sectors. There is a reduction in unnecessary repeat examinations, there is no loss in information and the patient knows his personal urologist. This is especially necessar...

Abstract The importance of data and statistics to support the political and tactical goals of the professional medical associations is constantly growing. Statements about the real medical care situation, including investments, operational expenses, use of labor and (medical) effectiveness, economic validity etc., should be reinforced by data and real-world evidence. More and more, in future arguments about the development of medical care, the party with the better data will prevail. That is why the umbrella organization of the medical specialists' professional associations SpiFa-Spitzenverband Fachärzte Deut...

[Molecular tumor board penile cancer-a challenge].
Abstract Due to its low incidence there is only very limited data concerning molecular markers in penile cancer. Recent studies show potential prognostic markers for lymph node metastasis, survival and response to chemotherapy or targeted therapy. Nevertheless the number of patients in the studies is very limited. Therefor clear recommendations for clinical decisions remain very weak. Patients with metastatic disease should be treated in clinical trials with translational biomarker research to improve the molecular tumor board in the future. [Urology, gynecology, and andrology united against infertility? : The careers of Boris Belonoschkin (1906-1988)].
Abstract Male arrogance, medical ignorance of anatomy and functioning of the human genitals, and social reservations about sex education have hampered urological and gynecological research since the 1920s. This changed under the premises of National Socialism when some physicians willing to cooperate were given the opportunity to perform human experiments. The gynecologist Boris Belonoschkin was one of them. He continued his career in Sweden after 1945, where he became an internationally respected scholar. As such, he was able to bring books of his former German colleagues to international attention again. He...

[Workload, job satisfaction, Work Time Act-what is urology doing?]
n M Abstract Due to demographic changes, the proportion of people requiring urological treatment will continue to rise. In order to better address this development, we need a well-trained and motivated urological medical community. Not only is continued education in urology an integral part of this, but it must also be embedded into future-oriented, flexible work-hour models, taking into account the German Work Time Act. Thus, in order to obtain a better assessment of the current situation of urological physicians undergoing specialist training, work -hour models currently used in urological clinics must be evalua...

PJ Abstract The introduction of molecular targeted agents has fundamentally changed the treatment of metastatic renal cell carcinoma. A first wave of development was based on the improved understanding of tumor biology since the discovery of the importance of the von Hippel-Lindau gene as the key driver of the disease and paved the way for antiangiogenic agents. Of relevance is the overexpression of proangiogenic and proliferation-promoting factors (VEGF, vascular endothelial growth factor; PDGF, platelet-derived growth factor) as well as an overactivation of the PI3K-Akt signaling pathway: the target struct...

Abstract Tumor follow-up in patients with non-muscle invasive bladder cancer (NMIBC) is a weighing up between the morbidity associated with invasive diagnostics and the risk of tumor recurrence and especially progression. The risk stratification into low, intermediate, and high-risk tumors enables a risk-adapted follow-up. For individual estimation of the risk of progression and recurrence, risk calculators should be used. Follow-up is still based on cystoscopy, which is recommended lifelong for high and intermediate-risk tumors and for up to 5 tumor-free years for low-risk tumors. Urine cytology has a&n...

[Anal funnels and wonder pills-the dubious market for medical products in the Federal Republic of Germany until 1965].
Abstract The Federal Republic of Germany was the last European Country without a drug legislation and a law about drug advertising until the 1960s. Therefore, a broad medical subculture flourished. Various dealers sold drugs and instruments for curing psychosomatic illnesses, especially anti-impotence pills or aids to reduce weight. Thus, the interested client could treat himself/herself without visiting a physician or a pharmacy. Some physicians tried to reduce this grey market, but they were not successful. Abstract Proper indication, preoperative diagnostics and final choice of surgical technique in the operative management of female stress urinary incontinence are subject to somewhat irrational, trend-based changes. For various reasons (insufficient reimbursement, poor expertise, limited therapeutic spectrum) preoperative urodynamic tests are increasingly thought to be unnecessary and are progressively replaced by perineal ultrasound despite lack of evidence. Since the AWMF guidelines (AWMF: Association of the Scientific Medical Societies in Germany) for the diagnosis and treatment of stress urinary incontinence in...

Abstract Orchitis can be acutely symptomatic or chronically asymptomatic. Among the acute forms is the rarer isolated orchitis, which is of viral origin in most cases as well as the more frequent secondary orchitis, which is usually the result of an ascending bacterial epididymitis. In addition, sterile forms of orchitis are also seen in patients with systemic autoimmune comorbidities. Chronic asymptomatic orchitis is the term used to describe cellular immune infiltrates in the testes, which are observed in approximately 25% of cases of azoospermia during testicular biopsy. Abstract Prostate cancer remains among the most commonly diagnosed malignancies worldwide in men. In patients with low-risk prostate cancer, the risk of metastasis and mortality is very low; therefore, a tumor surveillance strategy can be used. The aim of active surveillance in prostate cancer patients is to minimize treatment-related toxicity without impairing patient survival. To maintain patients under acti...
