Management of Febrile Young Infants
Investigators from Kaiser Permanente and the University of California San Francisco conducted a retrospective study to determine the incidence rate of fever, assess variation in evaluation, and estimate rate and types of serious bacterial infections (SBI) among young infants. For the study, the investigators reviewed records of a cohort of healthy, full-term infants born into the Kaiser Permanente Northern California (KPNC) system between July 1, 2010, and June 30, 2013, and identifying those presenting for care because of fever when they were 7 to 90 days old. The medical records of the febrile infants were reviewed and d...

Outpatient Therapy of Pneumonia: IV Versus Oral
Researchers from multiple institutions conducted a retrospective cohort study using data from the Pediatric Health Information System (PHIS) to compare the effectiveness and treatment-related complications of antibiotics delivered orally or through a peripherally inserted central venous catheter (PICC) after discharge among children with complicated pneumonia. Children were eligible if they were ≥2 months and <18 years old and were discharged between 2009 and 2012 from a PHIS-affiliated hospital with complicated pneumonia, defined as an ICD-9 discharge diagnosis code for both pneumonia and pleural effusion. The main ...

Pediatric Migraine Therapy: Illusive and Elusive
Researchers from multiple institutions conducted a randomized controlled trial to compare the effectiveness of amitriptyline or topiramate to placebo in reducing pediatric migraine headache. The investigators recruited patients aged 8–17 years from 31 US sites. To qualify for the study, participants must have been diagnosed with migraine or chronic migraine without continuous headache, experienced ≥4 headache days during a 28-day baseline period, and scored 11–139 on the Pediatric Migraine Disability Assessment Scale (PMDAIS), which is indicative of at least mild disability related to headaches. Once enrolle...

Ten Days of Antibiotics for Acute Otitis Media in Young Children
Investigators from multiple institutions conducted a randomized noninferiority trial to determine the effect of the length of antibiotic treatment for acute otitis media (AOM) on clinical failure. Children were eligible if they were 6–23 months old, had ≥2 doses of pneumococcal conjugate vaccine, and were diagnosed with AOM. AOM diagnosis was based on 3 criteria: onset of symptoms within the preceding 48 hours that parents assigned a total score of ≥3 on the Acute Otitis Media–Severity of Symptoms (AOM–SOS) scale (with high scores indicating greater severity of symptoms), the presence of middle-ear ...

Early Childhood Respiratory Infections: How Many Is A Lot?
Investigators from Finland conducted a prospective cohort study to characterize the clinical manifestations, early risk factors, and outcomes of children ≤2 years old with recurrent respiratory infections. Parents of newborns were recruited soon after birth to participate. Enrolled parents were instructed to keep daily diaries of respiratory symptoms of their child until age 2 years; parents also recorded physician visits with diagnoses and treatment, as well as absences of the child from daycare or the parent from work. Demographic information was collected at enrollment and updated at the child’s age of 13, 18, ...
