Improving outcomes in community dwelling older adults: An NHS and third sector collaborative approach

Purpose: This project was undertaken to determine whether the collaboration between an NHS therapy-led rehabilitation service and a third sector volunteer-led team would improve outcomes for community dwelling older adults in the local population. Keep Active is a third sector charitable organisation, who work alongside the Community Independence Service (CIS) to provide extended rehabilitation for community dwelling patients, in Hammersmith& Fulham.
