Reducing Vasopressor Exposure in Patients With Vasodilatory Hypotension —Reply

In Reply Dr Das and colleagues raise a number of concerns about the 65 Trial. The explanation for the median blood pressure being higher than the MAP target relates, in part, to the protocol. When MAP values were above 65 mm Hg, the protocol stipulated that vasopressor dose rates should be reduced until, if possible, they were stopped. As such, it was possible to adhere to the protocol by continuously reducing vasopressor doses and still record MAP values above 65 mm Hg until stopping the medication, at which time MAP values were not recorded. MAP values without vasopressor dose rates are not good indicators of protocol adherence.
