Is TEVAR an Optimal Strategy for Retrograde Type a Intramural Haematoma?

Ryoi et  al.1 report the first successful experience with mid term follow up of endovascular thoracic aortic repair for patients with acute retrograde type A intramural haematoma (IMH) with intimal tear in the descending thoracic aorta. In hospital mortality for type A IMH is 40% for medical therapy and 24 % for surgical therapy.2 Open surgery for retrograde type A IMH requires a more extensive aortic replacement to repair the distal intimal tear, which carries higher surgical risk.
