Isolated Omental Leiomyoma without Prior Surgical History

A 30-year-old, gravida 0, female was referred from a local clinic due to persistent, vague abdominal pain and a presumed pedunculated myoma at posterior uterine wall, measuring about 7.2  × 6.0cm by ultrasonography. She denied any prior surgical history. Laparoscopy revealed a mass entrapped within an omentum covered with engorged, serpentine blood vessels that traced back to a single pedicle (Fig 1A). Exploration of the omentum uncovered an isolated myomatous mass (Fig 1B). Af ter careful en-bloc resection, the specimen, along with another excised subserosal myoma (about 1cm in size), was removed via posterior colpotomy.
