Improving End-of-Life Care using the Life Sustaining Treatment Decision Initiative in a Veterans Affairs Community Living Center
In 2018, the Department of Veterans Affairs (VA) implemented the Life Sustaining Treatment Decisions Initiative (LSTDI), a national, multi-pronged program to support clinicians in conducting Goals of Care Conversations (GoCC) with seriously ill Veterans and documenting them using a standardized note and orders template. Addressing GoCC is a crucial part of comprehensive care in LTC facilities and thus Veterans residing in VA nursing homes (Community Living Centers, CLCs) are appropriate recipients of this initiative.
