Discussion with the lecture attendees will focus on overcoming barriers to change, adapting to increased demand, and whether full capitation payment can support the PCMH. Upon completion of this session, participants should be able to: 1.Complete the session clearly understanding the strategic and leadership and frontline processes and tools used to implement major change in payment model for an academic practice that serves as a "Safety Net" patient-centered medical home at a time of rapid expansion in insured populations 2.Bring out of the session a better understanding of specific LEAN and practice management tools and their situational uses 3.Complete the session with specific ideas of how payment reform can challenge a delivery system, change and support the PCMH delivery model, including the impacts on patients, teachers, and learners, as well as physicians, other providers, and staff 29373
