Disagreement: Should the duration of primary hyperparathyroidism impact guidelines for evaluation and treatment?

We reviewed the article by Randle and Lee.1 We agree thoroughly with the authors that greater duration of primary hyperparathyroidism (PHPT) should prompt us to evaluate end-organ involvement even in normocalcemic or asymptomatic PHPT.2,3 But we believe that the treatment planning should not be tailored according to the duration of PHPT. We have the following comments to support our disagreement with the authors.
