We present a case of CAA-ri in a subject already on immunosuppressive therapy after orthotopic heart transplantation (OHT). A 57-year-old man 8 months post-OHT for sarcoid cardiomyopathy developed headaches and staring spells while hospitalized for disseminated mycobacterial infection. We present here the case of a patient transplanted due to end ‐stage arrhythmogenic right ventricular cardiomyopathy (ARVC), fulfilling the task force criteria. A few years after successful heart transplantation (HTX), the patient developed similar symptoms and morphofunctional changes of the heart, which led to critical re‐evaluation of his primary diagno sis.

Sarcoidosis is a systemic disease characterized by non-caseating granulomas of any organ. Hypercalcemia is a complication of sarcoidosis, and a cause of acute pancreatitis. However, there are few published reports of hypercalcemic induced pancreatitis secondary to sarcoidosis. This case describes a patient with known sarcoidosis who presented with acute hypercalcemia-induced pancreatitis and eventually developed chronic pancreatitis.

ConclusionsWidespread grayish-white outer retinal spots associated with hyperautofluorescence on FAF and disruption of the EZ on OCT are not pathognomonic for MEWDS. A high index of suspicion must be maintained for masqueraders of MEWDS, which can include serious inflammatory, infectious, and neoplastic disorders.
