non-medical leave of absence effect on residency competitiveness

If an M1 student takes a required leave of absence (suspension) because of a combination of academic and professionalism/behavioral issues (not because of health) and starts again exactly a year from the point which they left off, what is the effect on their chance at being accepted into competitive residencies (say on the level of ortho or one of the 5 year integrated surgery+fellowship programs)? Do they basically have no chance of getting into anything more competitive than family med? Or... non-medical leave of absence effect on residency competitiveness
