Telehealth Intervention with Emergency Medical Services Personnel for Fall Assessment

This quality improvement project was done to reduce unnecessary emergency department (ED) transfers after a fall by providing a telehealth visit between an advanced practice provider (APP) with the onsite Emergency Medical Services (EMS) personnel. This impacts long term care patients because avoiding ED visits results in improved continuity of care, reduces the risk of hospital acquired infections, enhances patient comfort, cost savings, reduces unnecessary tests and treatments, and frees up ED resources.
