Disseminated peritoneal leiomyomatosis (DPL)

A 40 year old woman presented with heavy menstrual bleeding, dysmenorrhoea, bloating and lower abdominal fullness. She had previous myomas removed by laparoscopic myomectomy using a power morcellator without containment for specimen extraction 2 years ago. She had an enlarged uterus clinically with an MRI demonstrating multiple fibroids up to about 14 cm and a right adnexal mass. She underwent a laparoscopic hysterectomy, bilateral salpingectomy and right oophorectomy. At surgery, disseminated peritoneal leiomyomatosis (DPL) was suspected with the entire peritoneal, greater omentum, rectal serosa studded with multiple tiny subcentimetric and large myomatous nodules.
