Are extra locking bolts or fibular plating more important in extreme nailing of distal tibia fractures? A cadaveric biomechanical analysis

Distal tibia fractures can be treated with intramedullary tibial nailing or plating [1,2]. Nailing may be preferred to plating because of its minimally invasive approach which may decrease operative time and avoid fracture devascularization [3]. Further, nailing may permit earlier weightbearing [4]. However, it can be challenging to achieve acceptable fixation when the fracture location is so distal that it only permits placement of one or two interlocking screws through the distal portion of the nail, and there are concerns for higher rates of malunion in these fractures treated with nails [4].
