Oblique fiber preservation in the prevention of GERD after peroral endoscopic myotomy

We read with great interest the article entitled "Conventional versus oblique fiber-sparing (OFS) endoscopic myotomy for achalasia cardia: a randomized controlled trial."1 This single-center randomized controlled trial from India compared conventional myotomy (CM) with OFS myotomy in peroral endoscopic myotomy (POEM) and found that the incidence of post-POEM GERD was similar in both groups. The authors conclude that OFS myotomy did not help to prevent GERD.
