Phase Contrast MRI Can Separate Conventional ≥ 50% Carotid Stenosis and Near Occlusion

Objective: Carotid near occlusion is a severe stenosis causing a diameter reduction ( “collapse”) of the distal internal carotid artery (ICA). This collapse can be moderate with normal appearing distal ICA (without full collapse, Fig. 1A,B), or severe with a threadlike distal ICA (with full collapse, Fig. 1C,D). Conservative management is recommended for symptomatic near occlusio n.1 However, the diagnosis is difficult, and near occlusion is likely underdiagnosed and undergo unnecessary surgery.2,3 The current diagnostic reference in studies has been systematic feature interpretation of angiography (such as computed tomography angiography – CTA).
